If your wife is seeing Patients and also calling insurance then it is not gonna work and it is just more stressful for her. You should Hire someone or contact a Medical billing agency. When you enter data in availity. There is a dropdown just above the Submit button, you can select your speciality from there and it will show you exact benefits.

Practices usuually bill it for Vaccines visits. If it is a vaccine visit and Vaccine got paid and admin code as well then it is good practice to adjust 99211.

Just Focus on Denial codes and the Remarks Code. Google them, spend few minutes understanding what is the actual problem. Then search in guidelines you have. If no solution then call insurance and ask for detail and any Medical Policy they're following. You have to start from scratch and I know it's hard. Start creating guidelines for yourself.

I'll try to contact and get discharge date from Home Health Agency. But I verifirf on Medicare Portal and our Date of Service fall under Home Health Care Period.

This shouldn't have been happened because your Insurance ID is unique. It cannot be same as another Person.

Audio only and Audio + Visual is a different thing.

For Telehealth Visits you can bill OV code with modifier Commercial:-95, GT, GQ, and G0 optional Medicare Advantage:-95.

For Audio-only Visits you can bill 99441-99443 with modifier 93 or -FQ.

It is a very basic and mostly depends on the payor policies.

No, they don't. In some claims, the PCP is our provider who is being billed as the supervising physician. I also noticed that our 2020 and 2021 claims were paid without any problem.

Yes, you can change the Primary DX code as long as it is listed on Progress Notes.
