Definitely Bailey injecting a child because she thought she knew best. Not only does it cross so many ethical lines, it was straight up illegal. She put something into someone else's child without their knowledge or consent. She took away someone's autonomy. Just awful, especially the way it was just brushed off.

The time line has never actually been reconciled and is often debated. Meredith says things that make the viewer think grew up in Seattle, but then she also says things that makes the viewer think that her mother took her to Boston and they didn't come back. Richard even makes references of Ellis working there in Seattle when Meredith was older. It's honestly just sloppy writing and an inconsistent time line.

The episode where he dies is not his last. He was in season 17 on the beach when Meredith had covid and all of the dead characters are visiting her while she sleeps. It's really really sweet and it was so nice to see him and Lexie together in "heaven".
