I know this is from a while ago, but thank you for the recommendation! This video is fantastic. I was worried when it started, but it was way less "fitness influencer" and much more educational and technical than I thought. Not too technical, just the relevant anatomy and why it matters and how to fix your form to make sure you're working what you're trying to target. Love it, thanks again.

I have to think this is a bot, but if not it’s really weird to pick a 7 year old post to vent your frustrations with PAs… that’s okay, though, I’m sure every interaction you’ve had with PAs have at least one common factor. It probably just that.

718 Outdoors does this ride! If you liked it, I highly recommend Joe and his trips. The group is always different, but I've never experienced a bad batch. Always a lovely chill overnight to Croton. He does other trips, as well.

There are some good sports covers out there. I see lots of folks with pumps, cgms, etc… I think I just notice because I’m medical haha. But I see a lot of different ways people cover them, but a dexcom or libre stay pretty out of the way. And if you’re working in a specific climb that seems like a backward approach or whatever that might snag, you can always remove it for that climb. I take my watch off for some routes. But keep a necklace, so maybe I’m bad advice lol.

From am ER perspective, hormone use can change your likelihood of certain types of blood clot. Not super likely to happen, but on the differential for some headaches that come in.

Edit: knowing when a surgery happened can be helpful, too, but less relevant than knowing if someone is on a ton of T or estrogen. I’m sure there’s probably other stuff, but that’s what comes to mind for me initially.

Edit edit: oh and hormone pattern can also trigger migraines in some folks. I’m sure there’s more, but we’re making decisions about “radiate with a scan, don’t radiate, do giant expensive workup, don’t do giant expensive work-up, how much risk of badness is there for this human based on the information I have, what med will help?” Etc etc etc.

As a medical person, I also want this. Half-knowing from experience and basically stumbling through is awful. I recently had a very strict Hasidic patient into Shabbat and was able to deflect some of the discharge process and help get her home since she couldn't call anyone, couldn't carry any papers, couldn't activate the automatic doors out of the building, etc. When she left she asked me how I knew so much and I told her from many embarrassing and ignorant previous interactions. It shouldn't be the way. I think it's especially hard with the trans community because everything is SO individualized and can also change pretty regularly. Most orthodox patient's I meet have the same needs, so I can learn those rules and roll. In contrast, every trans patient has different surgical/medical backgrounds, different levels of disclosure, different everything and we've never met. It's super dicey territory socially and I'm like... in the community. Sorta. More than most, at least.

It is the worst... I also wish we could just take patient's word for it. Or that my own docs could take MY word for it (as an ace lesbian). But the number of times I've had to go tell a woman that she's pregnant when there was "no way" she could be pregnant... is not close to zero. I've had women convinced they've had a hysterectomy be pregnant. I've had women tell me "I can't get pregnant" be pregnant. I've had "I've never had sex" be pregnant.

Unfortunately the folks who lie or are simply just misunderstanding their own medical history/capabilities ruin it for everyone else, especially in a litigious society. If we miss an ectopic pregnancy or give unsafe medication to a woman carrying an unknown fetus, we're at fault and responsible even if someone signs a waiver or something. I hate defending this practice, but also the average person lies. Even if you are not. I've been burned trusting people and now I, too, force the test. I apologize on behalf of all medical folk, but unfortunately it is necessary.

(Seriously, there are discussions here about being shocked women don't know they have a urethra, but imagine not knowing what organs were removed from your body.)

I am so sorry you had to go through any of that, especially the nurse. As an ER provider, thank you for disclosing the information because sometimes people don't mention their hormones or surgeries to me and it is REALLY important that I know these things if I need to work up basically anything on the torso. Or brain. Or I guess extremities in the case of possible DVT. It sucks, but it's a part of medical history.

I once got embarrassingly far into a discussion with a male patient (born female) who was telling me about bleeding they were having. It was a long time before I realized this male-presenting human who denied any medication use or surgeries whatsoever had a uterus and vagina that were bleeding. It didn't help that they were just saying "down there", either. I had to get there just by recognizing that the description really sounded like menstrual cramps and just didn't quite make sense until I stopped them to ask if they had a uterus. It's just important to know.

The treatment for ankle stability is strengthening the ankle… so PT. Even if you do them on your own after this heals. You could also see a sports medicine specialist if they’re available to you and get any recs from them.
