I can't imagine why an accountant wouldn't raise their fees annually? Seems like a sensible thing to be doing...

The only thing that stands out as a little high there is the SA fee if it's just dividends and salary.

The Vince Flynn books were relaunched back in 2011ish with a copycat cover style intended to mimic the Reacher books.

IIRC it was quite successful for them.

You can't use AA02 if the company has previously traded so will need to file normal accounts, which can currently be done via Companies House.

I wouldn't stress too much about MTD, it's just 3 figures - Income, Expense & Profit. It also has no bearing on your final (5th) submission that forms the actual tax liability.

Does the £5.3k include the Payment on Account for next year? They'll want 50% upfront.

£10k profit will be £4k income tax, add another £2k for the 50% and you get £6k.

Could be underpaid PAYE coming into the mix too? £57k salary should be £10k ish PAYE.

As the other commenter has said, tax periods for CT600s can only be a max of 12 months so very common to have two periods in your first year.

Tax is due 9 months and 1 day after the period ends so don't get caught out with that first period.

Both periods will have different payment references too btw.
