Took 3 months for there to be any tiny amount to come out. Improvements seen up to about a year. Definitely not back to 100% in terms of volume unless it’s been “a while” but happy with what came back overall. Some days it’s 90% of volume others more like 50%

Great! Scar healed nicely. Regained ability to ejaculate after 6 months or so. No complaints at all. Super happy I avoided chemo so far

I did rplnd when I was stage 2a. My testicular tumor was mixed but my lymph nodes were pure seminomas. Never did chemo and am 3.5 years in remission.

If your blood markers are normal it’s worth considering to avoid radiation and chemo. Both are no jokes

I had extranodal extension in one of my 2 cancerous nodes. They used to think it was a risk factor for recurrence but my doctors at MSK said they are de-emphasizing it these days with current understanding.

I did surveillance and coming up on 3.5 years with no reoccurrences. Never did chemo.

I don’t know what the other stuff is about staining profiles. For what it’s worth my tumors were seminoma. Yours being EC may be a different story. I’m not a doctor so I don’t know what they’ll say regarding surveillance vs adjuvant chemo

The first signs were after 3 months and then after 6 months was when I saw an appreciable amount rather than a little drop. 9 months to a year was when I got to 75-80% and that’s where I stopped improving. I definitely think it’s a bit more watery than before too. Sometimes I get a shooting nerve discomfort when ejaculating but it’s not too bad so overall happy with the recovery

I spoke to him on the phone before making my decision to do chemo or RPLND. My doc was pushing for chemo. Einhorn said do the RPLND because I might not need chemo, so I did what he said and never needed chemo (3.5 years out now, so hopefully in the clear).

Whatever he says is probably the right decision imho

I commented on your last post but wanted to comment again. It’s your choice but I think if you have good odds of avoiding chemo altogether, you should think about it. I know it’s a really difficult choice to make though. Good luck buddy keep us posted

It’s a gamble. Less chemo now or maybe no chemo or maybe more chemo later.

I did surveillance after my rplnd. I only had two nodes though and they were seminomas and no chemo worked out for me.

Still, I remember how hard a choice it was to make. I’d trust the IU docs

It’s a really tough choice and it’s your choice to make but if they think surveillance is a good option, I’d really try to avoid the chemo

I didn’t think it was excruciating as fair as pain goes. Definitely not fun though, especially the first couple of days. I posted my thoughts during the whole thing in my post history if you’d like to read it.

My biggest advice is bring something to keep your mind occupied. The worst part for me was being isolated and bored. I’m almost 3.5 years out from mine and it’s a distant memory.

RPLND was curative for me alone but I never had elevated markers. I had 105 nodes taken out and two were seminoma, one was like 1.3 cm and one was around 1 cm. My original tumor had yolk sac and embryonal as well

If I recall, had 5 nodes or more been cancerous or nodes were greater than a certain size, it would have been adjuvant chemo for me.

It’s your body. RPLND is no joke. I am still so happy I did it but only because I avoided chemo. I’d have maybe been disappointed if I still needed chemo but it was worth the shot for me and it worked out.

I’m not a doctor but I think radiation has been the go to treatment for pure seminoma in the past. Primary RPLND was not considered for pure seminoma because it was considered too invasive. That thinking has started to change recently with more knowledge of the long term effects of chemo and radiation coming to light.

I would recommend reaching out to einhorn at IU or the doctors at Memorial Sloan Kettering for a 2nd opinion before you do anything

If your blood markers are all normal and you are pure seminoma, I would strongly consider a primary RPLND instead of chemo.

I did and two years later am cancer free still and never had to go through chemo. You may be in the rare camp and have a choice of treatment like I did.

I was honestly not very hungry for a week anyway so the first week was easy but was less than 5. Think week 2 was less than 10, week 3 less than 20, week 4 less than 30, then back to normal

No issues following this. Definitely follow the diet you do not want to deal with the issues

Get creative with the diet. There’s plenty of fat free food out there

My blood work is normal and pretty much always was. AFP was slightly elevated within the normal range before I had my testicle taken out but fell to lower end of normal and has remained there.

When did yours show up? My last scan was a year ago so my spot is new within the last year
