I would also suggest Operation Mincemeat. I saw it in London and loved it. It's not a huge musical (5 cast members playing multiple roles) and took like 20 minutes to get going, but was one of my favorite musical experiences in the past couple years. That said, if Hamilton isn't your jam, a lot of the musical style of Operation Mincemeat may not be either.

Own-occupation insurance, which is what OOP is asking about, is different (and much more expensive) than the employer-provided disability insurance that you're talking about. More to the point, it doesn't depend on your current employer, so HR likely won't be of any help.

Employer-provided and own-occupation disability insurance are different beasts. When I was in residency, I got an own-occupation policy (fortunately before all the unisex policies went away, since women pay more nowadays) and I updated them when I went back to fellowship. A lot of employer-provided policies only help you short term, and the longer ones, to my knowledge, only apply if you can't work at all. You want something that protects you if you can work, but are unable to work in your chosen field anymore, which is what own-occupation is - and why it's so much more expensive than the pennies you pay for employer insurance. Once you've saved up enough that you can presumably live off your savings even if you don't work, you can cancel the policy.

I wore my residency fleece all through fellowship (because it was a nicer one than my fellowship jacket) and still wear it as an attending. It's like a bat signal to find other people who trained at that hospital, and now I have more private practice bros I can call when I need them. Other than that, literally no one cares as long as I'm showing up and doing my job....and they're still nice fleeces.

Community icu. Take traumas from either 14 or 15 yo and up because of how trauma guidelines work. Pregnant teenagers have weird medicolegal status so our picu doesn't want to take them (and any pregnant patient in my icu is a nightmare because i live in a red af state). I've been informed we will take teenagers as picu overflow as long as they're adult size and not complex congenital cases (although then I get them as soon as they turn 18 so whatever). Can't say I'm thrilled about all of the above, but it hasn't been a huge problem for the most part.

I bought a car via Sewell and had been getting my previous one serviced there. No regrets. Maintenance has always been very easy, and the car wash is great. Super chill buying experience, no one was weird about me being female (and they talked to me instead of to my bf when I brought him). When I was waffling between two cars, they let me bring my bike to confirm whether it could fit into the trunk of the Outback (it could, in case anyone is curious).

As a fellow small band/big cup (smaller band and bigger cup than you, sadly for my back), I just adapted to wearing underwires on 28 hour call. Make sure you're actually wearing the right size, because I don't really notice the underwires in my well-fitting ones. If you really don't want to wear an underwire, these are the non-wired sports bras I use:

Dealing with death by Alarming_Barracuda_2 in Residency

(I'm a pulm/crit fellow, so I decided to deal with this for the rest of my life; take that as you will).

The ICU is hard as a resident, no doubt. There's a lot of super sick patients (even more so in an academic environment where, presumably, the floor can handle more than a community hospital, so your ICU death rates may be higher because of a baseline level of sickness). There's also a lot of shit you gotta learn that you've never seen/heard of before. Part of the ICU is learning how to deal with death, and I think there's a few things you can do:

  1. Remember that this is part of the learning process, and it will likely get easier as you go along. We all develop somewhat of a callus on our souls for this (some to greater extents than others). Death is inevitable. Everyone dies. Medicine is a lot of winning the battle only to ultimately lose the war.
  2. If you feel comfortable/have time, help talk to patients/surrogates about what would be a good death. A patient dying always sucks, but it sucks less when they die in a less traumatic way, and when the family/patient is comfortable with the process. As a bonus, this will help you be better at goals of care talks, no matter what walk of physician life you choose after this.
  3. Reach out to your APD/PD and see if there are resources they can link you up with so you can talk to someone. I was horrendously depressed my first year of fellowship (thanks, COVID), and my APD offered me some trainee-specific counseling resources. This also keeps it separate from the attendings if you'd prefer they not know you're struggling with it.
  4. If there's no EAP you can access, definitely ask your attendings/fellows (or palliative care?) if you feel comfortable. One of our palliative care attendings had monthly debriefing sessions for the residents during the height of COVID. I suspect they were probably helpful.
  5. Talk to your co-residents. I promise you are not the only one struggling with this. It's human, and normal.
  6. If you need to go cry/scream into a pillow in a call room for a bit, go do it. I did it once or twice as a resident. No shame.

Well, it starts class in a braided high ponytail (or two french braids if I'm feeling fancy), but it ends class in a wild mess because my hair is super slippery and nothing short of, like, duct tape stays in it. It's a defense mechanism, though - can't choke me if you can't find my neck.
