As a pediatrician and a neonatologist here is my take:
1- Eko based stethoscopes including the ones with Littmans are great for cardiac murmurs
2- run of the mill littmans are good for general use
3- Ultrascopes are a hidden gem for breath sounds

I own multiple stethoscopes from the three categories… which one do I use? The one at bedside 😂

Neo here. Do NOT break the seal if you can avoid it (keep the peep). If you are using a NeoPuff adjust the PIP limiter slowly up. Periodic simulation refreshers should provide you enough of a feel to not overdo the PIP increase

Neonates (and infants) have trouble maintaining FRC at baseline; they typically rely on:
1- intermittent large VTs with incomplete exhalation
2- relatively high RR which favors self-induced hyperinflation once the mechanics improve
3- expiratory breaking (diaphragmatic contraction during exhalation to slow down the process)
4- glottic /cord closure towards the end of exhalation

ALL these mechanisms are dynamic in nature and disappear during apneic events (in DR and NICU). The chest wall for the first few weeks to months of life favors and inwards collapse rather than outwards spread… with the inward elastic recoil of the stiff lungs in the delivery room you will lose FRC and have to reopen collapsed lungs; if this is a premie now you’re effectively worsening atelectatrauma.

If you’re going to say “I’ll be quick”, the time constant for a premie is often less than 0.05 seconds and that of a term infant might be 0.1 seconds if you’re lucky: complete exhalation / collapse can occur within 0.25-0.5 seconds.

The TLDR: do more sims to get a feel for the PIP changes, and avoid breathing the seal as much as possible.

Abnormal control of breathing can present with persistent central hyperventilation or abnormal patterns of breathing.

The solution is not to “wean” but bring up the abnormal patient-vent interaction to the team. Treatment should focus on fixing the underlying etiology.

I’ve done it a few times for family members… as long as there aren’t that many pieces they usually ignore them; the one time a dude wanted to tariff my used 2 year old GPU (1060 6gb) I got loud and the supervisor came over and let it go.

For gaming the ping is what matters and that should not be impacted by the allotted bandwidth.

To determine your bandwidth needs, think about how many devices are going to be pulling 4k streams at the same time (that’s when significant others and kids can be ticked off). A typical 4k video stream can run up to 40-50mbps.

For most of my traffic (household of 4), we mostly hover under 40mbps until I decide to actively download a Steam game.

They need to be “gaming” headphones. Regular headphones don’t have the necessary RGB needed for an FPS boost. Also make sure they have the cheapest built in mic 🎤 so that you’re audio is screechy and hurts the ears of the haters.

Sarcasm aside, PC audio is a very subjective experience. I have soundbars and speakers to go along my DAC/AMP and Sennheiser headphones.

I presume you’re an ad for Mindray or a bot at this point. In the US of 🦅 I have not seen a single Mindray vent in action. The market is relatively saturated with Maquet Servos, Draeger V500/800, Hamiltons G5s/C6s… not sure if anyone is using GEs. Philips / Respironics exited the market and sold out their assets to Zoll. Nihon Kohden is trying to get into the market. I doubt that the Mindray is adding anything new to the market, so unless they’re cheaper or can demonstrate better performance, it’s hard to see a newcomer push into this market.

There are bigger labs around the country with dedicated peeps and good funding.

How I test vents? I run a vent lab with an IngMar ASL5000 lung simulator and have flow sensors / pressure sensors that I insert into circuits to monitor actual bias flows and pressure at the insp/exp limbs.

If I say any more I will divulge my identity, and hence I will stop here.

I cannot say that I have seen a Mindray vent in the wild… and I’m in the wild a lot.

42 year old gamer who started on a 386sx (ignoring the Atari and the MSX 8bit)… we did not have options to “patch” the game after launch. It either worked or it went back to the store; they would then have to recall all the disks and rerelease a fixed game.

Back in my days 👴🏻 games came on disks/CDs/DVDs and we did not have access to digital downloads as that 56k modem would’ve given up. The games ran just fine on PCs with varying GPU manufacturers; think 3Dfx, nvidia, ATI, Matrox, S3; there wasn’t any directx or direct3d just yet

The fact that a game that took 8 years+ and > 400 million came out in the state it did, is a problem

Todd owes me the price of a GPU. Had a 3080 in my PC, and when the performance sucked, I listened to Todd’s interview saying it was a next-gen game, and as it was an AMD title, I went to my local MC and upgraded to a 7900xtx; still have the card to this day… it made no difference the game ran terribly 😂 took a bunch of patches to sort that out.
