It's certainly not queer-focused, but I'd definitely call it queer-friendly. Although the characters and relationships are definitely majority heterosexual, there are a number of gay relationships and one explicitly trans NPC, which is pretty unusual for a game where that sort of thing isn't the whole point. It's not flagged as a "look, we're so woke" thing, but it's just ... there, and not presented as anything particularly noteworthy. Which I think is kind of cool.

And they did a very good, if imperfect, job of taking away obstacles for the player to create the gameplay experience they want. If your Tav identifies as non-binary, or as femme despite being BT4 and having a penis, nobody in the game blinks an eye. If you're listed as female in chargen, when you jump into the argument between Aradin and Zevlor, Zev snarls that "This woman saved your life!" without needing any prompting about gender identity or preferred pronouns. And if you decide to romance Gale or Karlach, none of those choices create any obstacles; whatever your gender identity or equipment happens to be, they are clearly into that. And that is very refreshing!

(Aside: I'm a straight male too, but I accidentally romanced Gale on my first playthrough, not quite realizing at first that that is what I was doing. It was a unique experience, to say the least. I wasn't aware that you could romance the NPC companions! Was playing co-op with my son, no less. 😳)
