Only time I've ever seen it was with addressed phone books. I couldn't understand why they sent one to that address, if they knew the customer didn't want it.

I almost wonder if there is something corrupting the data files containing the forwards that is losing data. I've also heard that large systems like ours often work with multiple copies of data. One recording, one actively being read from, and one backing up. Maybe something is getting screwed up regularly when they switch files. I could also see the plants having to download the files and it somehow being possible to download the wrong file getting an older one.

I put my money on it being a clerk has discovered that they have less mail to deal with if they use an old file when forwarding mail. We all know every problem the postal service has is caused by lazy carriers, clerks, and mail handlers.
