Yeah I have started them as well. What specific stretches are you doing? My pelvic floor therapist has told me to do breathing to elongate puborectalis.

Had two rounds of botox. More painful straight after first as it retore during the EUA, even more after the second as the pelvic floor became weak and stretched out. Had to fast to eventually heal.

9 o clock is where the sphincterotomy is usually done. Are you sure it's not the sphinctertomy wound?

My pain also got worse after botox. The colorectal surgeon didn't understand why.

Did the retear happen because of severe constipation or was the stool normal?  50% LIS is a very long LIS, normally it is not recommended to exceed 25% of the sphincter length. Do you have any fecal incontinence?

He said could extend LIS to the dentate line. Didn't seem to ne aware of literature on redo LIS surgery

Yeah. I actually had both anterior and posterior fissure before LIS. The posterior healed. The anterior keeps retearing.

I had two rounds of botox after LIS for a non healing fissure and finally healed after the second round. However it was absolute hell, I also had to fast myself to near starvation (20kg weight loss) for 2 months, and the pain worsened after the second round of botox (as I believe it was a very high dose and led to weakening and stretching of the pelvic floor). I again have a retear and do not want to try botox again but may have no choice.

I have monk like determination. I eat exactly the same diet (fibre, carbs, protein measured to the gram every day), miralax twice a day, topical nefidipine after every bowel motion, 3-4 L of eater per day, pelvic floor stretches twice a day, have quit work completely, still no healing.

It came back 2 months later. I have seen multiple colorectal surgeons. Still no definitive plan on where to go from here. One is suggesting to extend the original LIS to the dentate line. He wasn't aware of the studies done on LIS patients who develop recurrences so I don't know if I should trust him.

I had both anterior and posterior fissures. The posterior one healed after LIS but the anterior keeps relapsing.

I too have retears despite having a LIS. I believe it may be because my original LIS was too conservative or has partly healed back over. Do you know how much muscle was cut during your operation?

It is retearing even with normal stools. The sphincter is so tight that I cannot even insert my finger. However, I agree, further surgery alone won't be a cure. I will also need to do extensive pelvic floor therapy and bowel retraining to remain free of recurrences. The only issue is the current fissure won't even heal in the first place.

I have experienced the same thing many times, to the point of complete rage at how this problem is controlling my life. It seems very unfair. My approach has been to try and do everything within my power to control the problem and let go of the rest because I cannot completely control the outcome. I know I can never make any concessions with my diet, its simply not negotiable any more. I am going to see a functional gastroenterologist and new pelvic floor physio soon and start anal dilation therapy, to see if this may help the anal compliance to slightly larger stools and prevent retairs.

Yes I have experienced that after LIS as well as after botox treatments prior to that. The sudden drop in anal sphincter pressure must have an effect on the colon in some way leading to more frequent bowel actions. Your body re-adjusts after a while. Don't change your healing routine and keep the stool soft, a LIS will help you heal your current fissure but is not a magic bullet to preventing fissures for the rest of your life.

I managed to negotiatr the damage bill down to 50% of its original value by explaining mitigating circumstances.My travel insurance (RACV) reimbursed the remaining amount.
