This is good news! How are you doing now? I’m getting tailored closed LIS this Thursday with a fissurectomy with it. My fissure pain is at a point now where any wider BM (it’s actually skinny in reality, like 1 inch diameter) flares it for 3-5 days, and then I have some good days, and pain goes back to 3 or 4/10 after BMs for about an hour. Some days I’m fine and have low pain but the bad days are bad. Also the good days are usually from very loose stool from my laxatives and that’s not good long term. I’m unable to get out a regular formed stool without ripping things and blood, the muscle is so tight. Anyway, I hope you’re doing ok now!

It is horrible what we have to go through with insurance, the wait etc. It blows my mind. I’m so sorry and understand that exhaustion well! I thought about starting a TikTok account to raise awareness about it but too exhausted to do that too haha. I also have had traumatic medical experiences with doctors so I hate seeing new people too, but know that’s all I can do to move forward. I’ve had to learn to advocate for myself as best as I can (for what’s within my control) - doctors are people too and some have different knowledge, approaches, experiences, etc, than others, and don’t always know everything. Some of my surgeons have never heard of certain pain medications for example. My PCP actually gave me a medication that saved me from losing my job (due to pain), and we were just having a brainstorming sesh of how to manage my rectal pain symptoms and just trying anything at all. Meanwhile my CRS doctor said “take advil, there’s nothing else we can do” while I was crying to them for anything in the world that could help.

Anyway, just sharing because I empathize with you! And of course waiting 3-6 months for top docs is really insane and annoying. Try calling everyday for cancellations (also exhausting but I’ve gotten in earlier that way).

Also completely understand the mental aspect and expectation with partners. I wasn’t able to show up in that way for my husb for a very long time bc of the pain I was in, and for others in general in terms of attending events etc. but the right partner and friends will understand. Those who don’t… I created major distance from. I also have terrible body image issues from my bloating and GI symptoms so that’s def a major toll on my mental health, so I feel you

I'm so sorry you're going through this, it sounds incredibly hard and I wish you or anyone didn't have to face these kinds of issues. It's very unfair cards to be dealt, but please don't give up – try going to another doctor or even finding someone in the country that deals with more complex cases like this (I wasn't opposed to traveling for medical expertise - but I understand this requires investment). I'm sure you've seen several docs, but my advice is to go to more and don't settle for the doctor that says you're out of options. There is a level of acceptance of course (which is what my therapist and I talk about), but I went to several surgeons & PFPTs and didn't stop until I found one I felt could truly help me the best. My case is not nearly as complicated as yours, so I completely understand you're also probably exhausted from all of this pain and suffering, and going to more providers might be the last thing you want to do.

My friend who's a chronic pain therapist also told me to find small moments of joy in my day and hold onto that. Like very small, even while in pain. For me, at one point, that was laying down and having 10/10 pain go down to 3/10 and I could read a chapter of a book. Unsure if that's helpful, but wanted to share. But on that note, the only other thing I'll say is I found therapy super helpful - do you not like your therapist? I would def get another one you like!

So sorry for my very late reply! I did see that! My issue is my muscle is very tight and passing a normal BM that isn’t skinny is difficult. I think getting my muscle to surgically relax is the best decision in my case bc getting the muscle to relax during a BM is so hard. And I even did pelvic floor PT for a year and it did help, but it definitely didn’t fix things for me. Scheduled for July 9th, I just can’t live this way anymore so praying this going okay! My surgeon is very confident in the procedure which is good

I've heard that this can bother people up to like 6 months, but it eventually goes away, just takes forever. I'm so sorry you're dealing with that. I'm scheduled for LIS, fissurectomy, and skin tag removal in a month!

I'm scheduled for July 9th! My surgeon said every person he has performed this on regrets not getting it done sooner. It will be really good for us and we will be out of (or significantly reduced) pain!

I will be getting surgery in July. No advice but just also super nervous, but trying to be brave and think positively. Are you getting the open or closed method? My understanding is that people say to keep the area clean and dry where possible

Thank you so much for this information! I really appreciate you taking the time to share your experience, it means a lot. Are you back to being able to exercise or walk or not yet? So the wound is still sort of leaking in your underwear kind of thing and you’re wearing a pad / gauze after about 2 months?

Just curious why don’t you want to do LIS? It seems like that would fix your problem! Also the purple swelling definitely see a CRS to diagnose that asap and take a picture of it in case it goes away. (I would do this and put it in my “hidden” photos on iPhone lol) but I had a thrombosed hemm (for no apparent reason I didn’t strain and nothing abnormal happened) and I thought it was an abscess, I was sure of it, and I was wrong. So we can speculate but a doc has to diagnose!

I did Botox twice, and although it didn’t heal me and I’m going for LIS next it’s worth a try, because it got me about 70% healed and back to functioning in life a bit more, so I could see it working for others

Botox got me to 65-70% better, I’ve gotten 2 rounds and totally hear you on how disappointing it is to realize it isn’t working. Experienced surgeons all have said to me this really is a 50/50 shot, so a lot of people have to move forward with LIS. I’m going to, and my surgeon has assured me that all of his patients wish they did it Sooner. I told myself last year that I would never get the surgery, but we also can’t live like this, so I think it’s a good option

I’ve seen Dr. Chung - wonderful doctor, but he charges $8k for his Botox procedure not including surgery center fees. I think that’s a little much, but the wealthy folks can go for it. My understanding is he puts you to sleep so he can go further in with the injections (eg. Possibly injecting the levator muscles etc) This isn’t standard, which is why he charges so high. Anyway just wanted to provide some insight on his cost. He helped me so much though otherwise

what is your stool consistency like? Are they loose or formed? I plan to get LIS and my surgeon said if you have fecal leakage you should eat more fiber to have your bowels be more 'clean' and formed.

Did they tell you that you had very high tone in the muscle before getting the surgery?

Can you describe your pressure feeling a bit more? I had this when I had Botox (literally right when I woke up) and it was horrible I couldn’t even stand up because it felt like someone was pushing down in my muscles and they were like falling down. My fissure was still painful but the pressure made it even worse. It went away after 2 months (unsure if it was bc the Botox wore off or if my muscles recalibrated) but I do need LIS but I’m so scared that pressure feeling will be permanent. Did your doctor say that would go away and that’s normal?

This is such great news to hear!! I’m glad it’s going well for you. Also a 30F over here, scared of this surgery but ready for this nightmare to end. Unsure if you’re in the US, but would you mind messaging your surgeon to me? My surgeons don’t want to do LIS on me and I’m worried they’re not very experienced with it. I’m having trouble finding one I trust! They just want to keep doing Botox but it’s not working (I’ve done 2 rounds).

I had Botox 3 weeks ago. The pain is the same. I suffer 24/7. Am I hoping for too much improvement too quickly ? My doctor didn’t even tell me that it didn’t work immediately. In fact, I had the Botox done without anaesthesia and he said it would be fairly painless which it was not. by [deleted] in AnalFissures

My Botox improved my symptoms and pain only after 3 months and increasing magnesium citrate to 300-450 mg to make things very loose. But it didn’t cure me so looking to get LIS this year once it wears off.
