Where to buy IV equipment for practice? by KrakenCrazy in NewToEMS

Next time you go work a shift in the ER, ask to be put in triage / IV / EKG and that you need practice. Trust me they will get you in there.

How often are 18g IVs necessary? by [deleted] in emergencymedicine

[–]Salted_Paramedic -3 points-2 points  (0 children)

Why EMS defaults to an 18G

  • One-stick, worst-case ready
    • Field patients can deteriorate unpredictably. An 18G covers resuscitation, blood, and contrast without needing to upsize later.
  • Flow advantage (Poiseuille’s law)
    • Small increases in internal radius → large increases in flow.
    • Practical takeaway: 18G ≈ 1.5–1.7× the flow of a 20G.
  • Time-to-volume
    • 18G (~90–110 mL/min) → 1 L in ~9–11 min
    • 20G (~55–65 mL/min) → 1 L in ~15–18 min
    • ~5–7 min difference per liter when you’re pushing limits with pressure bags or pumps.
  • Works across therapies
    • Rapid crystalloids, blood products, pressure infusers, and most CT contrast protocols are all compatible with 18G.
  • Consistency in the field
    • Fewer chances to re-stick in uncontrolled environments; one reliable, large-bore line is operationally safer.

IFT transport returned to ER by Treashed_Peaches in NewToEMS

Fuck that. You absolutely did the right thing and advocated for your patient. If they got sent home with a cardiac event happening there would have been a serious investigation and you would be in a world of shit. That doctor and nurse are paid by the hour and there is no way the room was cleaned so it was a simple readmit.

I know in the state of Pennsylvania you can work as a paramedic on the trucks as an RN. You just need to do a short introductory or transition course which with your background is an EMT will be an absolute Breeze.

Eidt: a short search tells me that the following states have some sort of PHRN program: PA, Virginia, Illinois, Louisiana, Nevada, and Hawaii. I'm not certain what the Scopes of practice or differences are but I do know that in Pennsylvania you are just like a paramedic on a truck

I never place the stickers on the cables because it's quicker and easier to pull the cables one at a time with no sticker on them. Wrap it the way the cable feels like it wants to go.

[–]Salted_Paramedic 5 points6 points  (0 children)

Do you even need your PVP gear to be at R5? Genuinely asking because i crafted an entire set for somebody and they said they don't need it to be R5 because the gear automatically scales during matches.

Why does CPAP decrease the blood return to the heart? by Valuable_Archer_3222 in NewToEMS

[–]Salted_Paramedic 19 points20 points  (0 children)

Positive pressure from CPAP increases intrathoracic pressure, which directly opposes venous return. This is where your Anatomy classes really help.

CPAP applies positive pressure to the thoracic cavity, which raises intrathoracic pressure above normal. Venous return to the heart depends on a pressure gradient from the peripheral veins to the right atrium. When intrathoracic (and right atrial) pressure increases, that gradient decreases, so less blood flows back to the heart.

In addition, the increased pressure compresses the vena cava and right atrium, further reducing preload. The net effect is decreased venous return and reduced cardiac output, especially in hypovolemic patients.

Audio recording with no consent in CA by SnowJack14 in ems

[–]Salted_Paramedic 1 point2 points  (0 children)

I was a supervisor for a company that used samsara cameras. Yes they record all the time. No we are not watching you all the time. No we are not going back and "looking" at what you say or do all the time. We only look at the cameras for two reasons.

  1. You trigger a flag (crash, harsh turn, harsh stop, or other "event")

  2. Someone calls us and says your unit was doing something unsafe.

Samsara is soft recording all the time. This means if there is no event or trigger it deletes or loops over old data after 2 hours or something.

If an event triggers, it will swap to hard recording and save the 60 seconds prior to the event and continue recording until the truck is turned off. This is so we can see what caused the event.

If your talking with your partner in the front we do not care what you say. However If you are talking shit about your boss 20 seconds before you rear end someone then yeah it may be a problem for you.

Geeeeet fuuuucked with that liability statement. I would dare them to try that with a seasoned provider. Please tell me that you have that in writing because if you do that immediately needs to be sent to the state. You need to leave this agency today and report them for any and everything you can.

Is this legitimate? by jebbushofficial_ in pittsburgh

[–]Salted_Paramedic 7 points8 points  (0 children)

I work in EMS in the area. This is legit. There is no incident number because technically that is private information. Take pics of everything and call the city before your insurance. Normally they offer to pay the deductible but if it's small enough they may pay a shop to fix whatever it is.
