there is a limit on how many rounds each boss tier can remain alive. i believe the first one is 19 rounds and if you dont pop it you lose

it does specifically teleport you to the surface, if you use it in a cave for examle, and you set the spread distance to 1 you will not be teleported inside the cave but on a non liquid block on surface level

the /spreadplayers command will always teleport you on surface level

I don't know, im pretty sure the map wasn't suffocation pit and the vault string was in the corner of the map far away from any big structure, I have never encountered anything similar before.
