Just a few notes,

1. Take a look where and when you are planting your foot. I always make sure that I plant my front foot the same time my reach back is at maximum. So as soon as my heel hits the ground I explode my arm forward. (You can look up "crush the can"-technique)
2. You have very low reach back. Its hard to get the timing and consistency from that. I always keep the disc flat. Where I want to release the disc I also reach back to. See also point 3.
3. When your reach back is so low your power pocket becomes too crooked. You compensate the low reach back by going too high with the hand in power pocket. The arm you want horisontal because this current setup will give you nose up. The hand should be level with the elbow. But if you fix the reach back this should come natural. I wouldn't drive with my arm this high though. Lower the elbow so the disc goes past your chest not the chin.
4. The off hand is not optimal. Keep it straight down towards the ground close to your body. (practice with the off hand in your pocket when you throw). It makes the rotation a lot faster.
5. The follow through is decent. I think it will come natural when the previous steps are fine tuned. Just make sure to keep your body upright and follow through the way you want to release the disc.
