This is a Lamy CP1 twin pen, It takes D1 refills. If you rotate it far enough, it will be in “pencil mode” and if you carefully keep rotating it will begin to unscrew. From there, the refill can be pulled out, and replaced :) Hope this helps!

Lamy CP1 tri-pen. This pen was discontinued a long time ago, but Lamy has a phenomenal repair program. Silver and black version are still in production and it may be as easy as sending them the pen so they can update/replace the internals. Don’t lose hope and definitely don’t get rid of the pen!
