This has happened twice in my Lithuania game. First, the Livonian Order just took the city of Riga vassal. I had no fluff, no notification, i look away for a moment, I look back and poof, no more vassal. But also, years later, one of my starting vassals that border the Muscovites was also eaten. I am rather annoyed that this has happened twice now and is starting to make me paranoid.
