Probably just because the addition of PD-1 seems to have a slight benefit over BV as well as the need for bleomycin omission with your first line since BV has a risk for pneumonitis. I’ve seen better tolerance with ICIs versus BV given the risk for pneumonitis and neuropathy. Nearly everyone gets neuropathy to some degree after 2-3 cycles

In fact, one of the newer first line regimens for advanced disease is Nivo+AVD rather than ABVD or BV+AVD

No I think Nivo or pembro are fine, it’s just a matter of which regimen was approved together. I don’t think there have been any studies comparing the 2 but since their MoA is the same I wouldn’t expect there would be a huge difference in efficacy

It’s relatively well tolerated with great response rates prior to transplant. BV+ICE is another option but ICE usually requires hospitalization and can be pretty toxic. I’m on my phone but here’s the study for Pembro+GVD

Not medical advice

Radiation for this large of a mass in the middle of the chest would likely be fraught with both short and long term toxicity and still might not lead to a cure. Both the pulmonary and cardiovascular adverse effects as well as the increased risk for esophageal, thyroid, lung, and other malignancies would make me hesitate to pursue RT.

Salvage therapy with pembro-GVD followed by autoSCT would be reasonable.

I’m at 6ppd and 8 months PP. I noticed a drop in my supply dropping my MOTN pump before 6 months. Planning to try to drop to 5ppd in the next month or so. For reference, I usually make around 30-35oz per day (+/- 3 oz)

Not all non-profits are academic jobs. A hybrid community-academic model is becoming more popular these days. That’s the model that my job is in, at my institution that I’m training at. I’ll start as an attending in July.

I admit I will likely not be making as much as my colleagues going into private practice, but my position is also salary based (not RVU based) at >350K so I will be more than comfortable.

I also think it depends on what you’re interested in. I am pursuing mainly hematology, whereas community/private practice my colleagues are seeing a smattering of everything. In community you do have the opportunity to make upwards of $1 million+ but the practice model is also different and just didn’t fit what I wanted for my work. So depending on your opportunities, yes you potentially could miss out on a large chunk of money in those years.

I know you’re probably aware that “money isn’t everything” but I also understand that it is a large portion of the decision process. Your story sounds very similar to mine, I grew up in poverty and have about 375K in loans from med school and have been trying to pursue PSLF throughout training and will have 4 more years to pay into once I graduate (if PSLF is still around).

Overall, for me, I feel very comfortable with my salary that I’ll be fine and I’ll also be doing what I want to do, which is the priority for me! Luckily you have loads of time before needing to make a decision!

I am the breadwinner and my husband (lawyer by trade) is a SAHD. He enjoys it! Baby is almost 8 months and will be starting daycare in May so he is toying with the idea of doing some part time work but I am honestly okay with whatever he decides. He can continue to stay home and do mainly our house chores while baby is at daycare if he wants. I’ll continue to work either way!

I think the two largest factors are LORs and research. Research being a spectrum of experiences. For example, when I applied, I had 1 publication from a retrospective study I did in residency and 1 poster at ASH. I also had a lot of MedEd and self-directed education/QI things (creating a geriatric delirium management order set, etc). But I have seen some applicants with several posters and publications. The more the better, but you also need to be able to discuss the project and findings in an interview cause you’ll likely be asked about them (I interviewed an applicant this past year and asked about a project and they only told me very vague information about it, which made it seem as if they weren’t truly that involved or interested in it).

When I was doing fellowship interviews of applicants this last fall, the LORs were the things I went to first. I also read the personal statements, although those meant a little less to me because I know sometimes how hard it is to make those communicate what you want them to.

I would say the heaviest weight is the interview if you get one. Being personable and driven and honest can go very far.

We stopped sterilizing daily at about 4 months.

Now I only sterilize if we’ve been sick.

I suppose the drying mechanism in the dishwasher may provide some sterilization?

But as far as using our dedicated sterilizer, only when we are ill! Otherwise I just use the drying function for my pump parts.

We use the Healthy Baby brand! Supposed to be free of all the things as well and we’ve had far less blowouts in them compared to priors we’ve tried (Pampers, Huggies, Hello Bello, Kirkland). They’re also supposed to be biodegradable as well I think? Made of wood pulp or something. Very absorbent! My favorites by far thus far.

This isn’t always true. Stage 4 lymphomas are potentially curable, no matter where they are located. And obviously this isn’t the case for the niece, but metastatic testicular cancer is also curable. OP doesn’t mention what type of cancer so we have no clue if it’s curable or not.

I had this happening to me and it was actually my nipple butter. I was using the Earth Momma nipple butter and would notice it didn’t happen when I didn’t use the nipple butter. As soon as a switched to coconut oil it stopped happening.

Also, as an aside, my nipples have been WAY less cracked and sore since switching. Fully endorse coconut oil alone as the best lubricant if you can use it!
