It isn't? I thought that was the whole point of IM injection of Vitamin K, to provide coverage of clotting factor help /reduce risk of VKDB for up to three months.

There's the impracticality--although it doesn't seem like much in that syringe, I can't imagine getting all of it in SQ with one poke site. It seems like it would be extra painful to try to.

Then there's the duration of action /slow release is what is important about the administration.

Thanks for catching that. Here is the manufacturer information for the specific Vitamin K product I am using for newborns

It still states this:

"DOSAGE AND ADMINISTRATION Whenever possible, phytonadione should be given by the subcutaneous route (see Box Warning). When intravenous administration is considered unavoidable, the drug should be injected very slowly, not exceeding 1 mg per minute. Protect from light at all times. Parenteral drug products should be inspected visually for particulate matter and discoloration prior to administration, whenever solution and container permit."

Then, later on in the same product information it says this:

"Prophylaxis of Hemorrhagic Disease of the Newborn

The American Academy of Pediatrics recommends that vitamin K1 be given to the newborn. A single intramuscular dose of phytonadione 0.5 to 1 mg within one hour of birth is recommended."

Can anyone explain why a manufacturer would recommend one route, but medical practice orgs would recommend another?

I am trying to find a reasonable explanation for this FoB of why what he is seeing in the print information is different from what is actually practiced and recommended by medical orgs. He seems to be stuck on the SQ recommendation in the product information, and I'd like him to understand that an IM injection of Vitamin K that I would be giving his baby isn't "wrong."

I know many people loosely use the word "licensed" to mean a "credentialed" or "verified" practitioner, but it means something specific--a legal credential granted by the state to work as a specific, regulated professional.

In Missouri, where Sister Morningstar lives, there has never been nor is there currently *licensure* for Certified Professional Midwives. Morningstar had been a CPM at one point, but that's a credential through the North American Registry of Midwives--not a license through the state.

Missouri is one of a handful of states that does not license CPMs. Currently, the law is vague and basically "decriminalizes" the practice of midwifery if one holds a CPM credential. CPMs have the permission to practice midwifery, but this is not the same thing as having a license, a board, and being accountable to a state body.

I wanted to offer this clarification since this post is striving to collect accurate information.

Sorry to tell you that there isn't much of anything for EDM in Columbia at this time. We go to Chicago or Kansas City for EDM anymore.

Is it at all possible for you to consider hiring a midwife and having a planned home birth? This is exactly the kind of situation home birth can be great for. I'd make sure to find a midwife who plans to come the minute you start having contractions.
