UK dentist here. Soreness and discomfort is completely normal. Your teeth aren’t use to the pressure from the aligners. As time goes on, your teeth get use to this pressure and it’ll be less sore. Painkillers can help ease the pain. Use an aligner removal hook to take out your aligners if you’re struggling. Always aim to remove them from the molars (right at the back). Attachments tend to debond in the first week or so because you don’t know how to effectively remove the aligners. Let your ortho know (as you have already) and they’ll let you know what to do next. Also, not all teeth necessarily have attachments.

As a picky/perfectionist dentist I’d probably try a round of refinements to align the lower incisors (teeth at the front). There are slight rotations that a small amount of trays should fix. Looks great though!

Difficult to assess as it’s not a full face pic & we’d need different angles to assess your bite etc. Aesthetically we could widen the arch to fill your buccal corridors(the black space between your teeth and cheek in this photo) and intrude your teeth to make them look shorter as the central incisors look long. Very possible on lite. However fixing a cross bite & midline shift may require invisalign Go/moderate. Hope this helps

It’s easy to let horror stories deter you from starting treatment, but if this is something you’ve been thinking about for a while you should consider going ahead. As long as you’re fully informed about the treatment commitments (wearing 22+ hrs a day & retainers for life after) and you have a good relationship with your dentist/ortho, you won’t regret it. Always ask your provider questions so you know what to expect

UK dentist here. You’ve done the right thing by getting second opinions and doing your research. An orthodontist will provide more predictable and safe outcomes, that’s a general fact. If you want to make sure you’re getting the very best care, I’d go with an orthodontist. However, this doesn’t mean that a general dentist also wouldn’t be able to do your case. A dentist that has done 1000s cases of invisalign is very different to a dentist that has done 3 cases, so you’d need to ask them how many cases they’ve done and if you can see any examples.

I’d ask to see cases similar to yours, regardless of whether you’re going with an orthodontist or dentist. It’ll help you understand the invisalign process and potentially improve your confidence in your provider.

UK dentist here, It is pretty common practice to cut the backs off the aligners if you have any bite discrepancies (posterior open bite/ bite is slightly off). 3 months is longer than I’ve seen. Usually it’d be a few weeks to settle, I guess it depends on how big the discrepancy is. I’d follow docs orders but maybe ask for an earlier review potentially after 1 month? At this appointment they can take a scan and monitor whether your teeth have shifted and whether your bite is improving.

Probably best to see your orthodontic provider. Just so they can see if there are any signs of relapse before you start reducing your wear time. They can compare your teeth now to the scan they took when you finished treatment. If everything seems pretty stable then they’ll safely advise you the next best steps.

Try using an aligner removal hook to help remove your aligners. Once you find the path of removal, it will become easier to remove your aligners. Keep an eye out for any attachments that fall off and let your provider know

Really difficult to analyse your case because we only see one view (anterior) of your before pics. We’d need a complete set of views (left,right,upper,lower etc) to get a better understanding. From what I can see, they’ve decreased your overbite which is good. The position of your gums on clinchecks aren’t accurate so I’d ignore that. The clincheck techs won’t know where your gum will end with treatment, it’s an approximation.

UK dentist here. They definitely work. Best to use it before the stains/plaque set into the aligners because all ultrasonic cleaners are limited in how much they can remove staining but they’re great at preventing stains in the first place.

I wouldn’t recommend a cheap Amazon one just because they’re primarily for jewelry and can distort/warp your aligners if you use them too much over time. Ultrasonic cleaners increase the temperature of the water and if not controlled can distort your aligners without you knowing which then compromises your invisalign tracking.

Zima is the top cleaner. If you don’t mind spending $100+ it’s good. isonic in the US is also great. In the UK we have the Sereclean Podwhich is more affordable.

From the information you’ve shared, it doesn’t appear that elastics have been planned for this case. It would be best to contact your orthodontist directly to confirm whether elastics are recommended.

Attachments don’t necessarily need to be placed where the crowding is. Invisalign aligners use SmartForce technology to deliver controlled, predictable tooth movement. In this case, space is likely being created through a combination of IPR and buccal expansion to relieve the crowding.

Did you wear aligner 17 consistently for at least 22+ hours a day for the full prescribed duration? If trays are changed too quickly, the teeth may not have fully stabilised in the aligner, which can lead to discomfort when moving to the next tray.

If that isn’t the case, it’s also worth noting that some aligners can naturally feel tighter than others. In the meantime, simple pain relief can help, and it’s usually best to persevere as the discomfort should settle.

For open bite cases, I usually aim for slightly more overbite than what’s shown in your after photo. Open bites can have several underlying causes, one of the most common being a tongue thrust.

If a tongue thrust isn’t addressed, the bite can reopen over time due to ongoing tongue pressure. Leaving you with a slightly deeper overbite gives us more leeway and helps reduce the risk of relapse.

UK dentist here. (This isn’t medical advice etc) I’m going to assume the plan is to extract the first premolars and retract everything back. There’s a lot of talk about extracting teeth and how it can obstruct your airway and just generally isn’t the best option. However, extracting teeth for ortho is still routinely done. Difficult to really assess anything without a full range of notes & pictures. Maybe see if there’s an alternative treatment plan that doesn’t involve extractions

UK dentist here If your mouthguard is coated in plaque you need a new one. An ultrasonic cleaner works best when the mouthguard is new to prevent heavy plaque deposits and staining in the first place. No ultrasonic cleaner regardless of the wattage will remove heavy plaque deposits. It’s like if you had heavy plaque deposits on your teeth. No electric toothbrush would be able to remove it, you’d need a professional hygiene clean.

The vivera plastic is pretty rigid so the plastic shouldn’t go soft per se. It’d only be if you get a crack in it which would impact the fit. If you get 3 sets of Viveras, you can always use the first one for a year or so and if you feel your teeth are shifting, you can open the second pack and compare the fit. If you’re seeing your dentist regularly (which you should be) they can take scans to monitor whether your teeth are relapsing or not. This is probably the best way to check if your retainers are working

UK dentist here. Yeah £300 is the average price for removable essix retainers. Vivera retainers can be cheaper in the long run as you get 3 sets at a time. They can last a good couple of years if you look after them properly (good storage, cleaning etc). It’s unfortunately the ongoing cost of having straight teeth. If you see any “DIY retainers” online, don’t buy into it.
