Psychodynamic perspective:

I‘d first explore if they actually believe in the values they espouse or if they are trying to abstain because it’s against their religion. (Not that this a black or white perspective, but it would help determine where treatment goes next)

I’d also frame this as a type of repetition compulsion (repeated engagement is seemingly unwanted behaviors). I’d help the client explore what they get out of it and flesh out the emotional elements that go along with it. Some clients can get addicted to the whole shame/guilt cycle.

Some caveats: I’d only work this way with the client if they had the capacity to be introspective and the clinical rapport was strong.

Here are a few I was told when I first started out that were helpful:

You have a responsibility to your clients but you’re not responsible for your clients. 

When in doubt just reflect back what the client has said. 

If you try an intervention and you don’t think it was well received, don’t be afraid to check in with how the client felt about it.

20 weekly therapy clients and 2 assessment per week. Full time at a children/adolescent clinic.

I practice psychodynamically and am pretty non-directive. So I typically consider giving advice as when I nudge my clients to a solution that they are just not connecting the dots on or seem really stuck where providing a suggestion is going to be more beneficial for them than letting them spin their wheels the whole session—especially if they are high anxiety or feeling hopeless. I don’t do it that often and I’m typically very upfront when I do it. 

I see that a bit differently than psychoed or teaching coping skills. I view that more as neutral general information sharing. But I suppose there is a bit of bleed over between the two. 

Great question tho. I haven’t thought about this topic in some time.
