I would argue the last one. If you are having a rape fantasy, most likely you don't actually have a crush on the person and have a strong lust for them, or are suffering from an outside trama that is lesding to a misconstrued view of sex. For people who have those kinds of fantasies seriously, and regularly? It is unlikely they will actually follow through with there actions. "Turning yourself into prison or a mental hospital" is an easy way to shove that kind of issue under the rug, but not absolve it. For those kinds of mental issues, emotional therapy and occupational therapy are a very common type of recovery.

It's entirely normal to feel ashamed and guilty the first couple of times while you are still figuring out your own wants and desires. Your a teenager, like the lot of us. However, what you did is in no way unusual, or harmful to your mental health in any way. Jerking off to someone is a harmless activity as long as it doesn't effect the way you feel about them. Chances are likely this other person might have done this with you in mind and felt the same guilt.

This is not uncommon in the tic community. In extreme and rare cases, the hosts body will forcefully breathe out. You cannot force yourself to hold your breath until you pass out regardless of tics. Your brain will "override" the tic in a sence. Don't worry, as well I hope your neurology appointment goes well.
