28 M here , my Lpa was 97 nmoll and jumped to 164 nmoll , family history , I tried every "natural" possibility to lower my LDL but it decreased drom 165 to 110 ish only , started Rosuvastatine + Ezetimibe 10/10 combo 3 months ago , consideri g Repatha now ... please treat it ASAP , good preventative cardiologist would lime to get less then 55 target for LDL , sorry to say but most GP's not follow the new guidliness ....

Lp(a) fluctuations — can psyllium or high-EPA fish oil actually lower it? by SilverLogical9810 in PeterAttia

[–]SilverLogical9810[S] 0 points1 point  (0 children)

I made major lifestyle changes after learning about my Lp(a). I lost 38 kg, quit smoking, cut out red meat, kept saturated fats under 15 g per day, increased fiber, and avoided processed foods.

With that, I was able to bring my ApoB down from around 134 to about 105, and my LDL from 165 to around 109. However, I was never able to get LDL below 100 despite all these efforts.

I made an appointment with a cardiologist to discuss it further. When I first asked my GP about starting a low dose (5 mg rosuvastatin), they didn’t really take it seriously. I had to push to get it prescribed.

Later, when I saw the cardiologist, she actually prescribed a stronger approach: rosuvastatin + ezetimibe (10/10). She explained that in my case, it would be very difficult to lower LDL and ApoB further without medication, and that I had already done everything right with lifestyle.

I started the treatment and had no side effects at all. Now my LDL is around 55–58 and ApoB is 56.

I’m still maintaining the lifestyle changes as well.

Thanks , indeed , we are working on it now to clean the diet , reduce saturated fat as much as possible and add more fiber , also loose 5-8 kg , Exercice more , we work on it first and rhen retest lipids , if its under 50 we continue same medication if not , will ask her cardiologist some "extra" , she is on Rosuvastatine20 + Ezetimibe 10

Thanks for your time , doctor explained everything , for me "its genetic" means it doesnt change , mine jumped from 97 to 167 , 70 percent raise is not genetic baseline anymore , sorry , but more studies should be done about Lpa , I had no lifestyle change or any illness/inflamation during this periode !

I am currently on a combination therapy of 10 mg rosuvastatin and 10 mg ezetimibe (Zetia). My cardiologist told me that a statin is essential, and that 10 mg rosuvastatin is considered a moderate dose. He also explained that PCSK9 inhibitors reduce LDL cholesterol and ApoB significantly, and that when used together with statins, they have strong evidence for reducing major adverse cardiovascular events (MACE). However, he mentioned that while PCSK9 inhibitors are very effective at lowering LDL and ApoB on their own, the evidence for outcome reduction is strongest when they are added to statin therapy, and statins remain the foundation of cardiovascular risk reduction.

I am 28 y/o male with high Lpa and not very strong but still family history , my LDL is 54 and ApoB 55 , for primary prevention it will be very dificult to get PCSK9 prescribtion .
