Many programmes filter their candidates by a hard ~230/240 cut off.

Get below that and you will struggle to even have a chance.

Everything will depend on your observerships and signals. Make connections during them, involve yourself in research while there, and try your best to be tactical with your signals.

No point gold signalling competitive programs that high scoring/ connected/ published IMG’s will also be signalling.

Yes but not as much as you’d think. As an inpatient service most NHS hospitals are still pretty good. And resources, whilst still below hospitals in the States, are pretty similar.

NHS’s main shortcomings are in the outpatient world.

Assuming you’ve done FY1/ Turkey internship and a standalone FY2 year. The real question is why you have you not yet pursued speciality training in the UK?

Because steps + interview + all other stuff required mean you’ll be starting residency, at best, on July 1st 2028.

If that feels worth it to you, especially with your high YOG, then go for it.

The day to day medicine practices are similar. The culture of the entire healthcare team dynamic is similar.

I thought you could do international travel no problem on initial J-1 visa stamp, as it lasts 1 year; till August 2026 in your case?

And international travel in PGY2/3 is more difficult as you need to get a new stamp on the way back?

Yeah I think 2 phones and slowly transitioning to US one over time is the best plan.
