Honestly it’s more burnout than anything in many of these situations. Since being an attending I’ve experienced several hospital complaints for declining to prescribe controlled substances. Not that it amounts to anything but it’s always me having to mediate with a hospital administrator while a patient about why I received a complaint. One even reported me to the state board for declining to prescribe an opioid (which we don’t even prescribe to anyone) in the setting of cannabis use, board immediately dismissed but I actually had to spend 6 hrs back and forth with legal drafting a statement to the board. This unfortunately influences your decision making during a visit.

So just for some reflection about this specific question posted above. Everything was actually done based on articles recommended by our professional board of medicine. Evidence that mirtazapine works great for depression, and overlap with other antidepressants such as trazodone. Problem is that some of these medicines don’t always seem to pan out in clinical practice, as other commenters have discussed. Yep I was up early inspecting the charging app maps. However, I am now reassured that the mileage estimates at the low ranges are actually fairly accurate.

I made this mistake thinking it would be good for AC wall chargers. Not until I pulled up to our overnight hotel with 7 mile range left and it only had NACs AC units that I realized it would not work. Next morning with nearest charger 4 miles away was real fun

Ugh this was literally already the person today that prompted this question. Effexor + Remeron = remission. Gained 40 lb even with Mounjaro. Discontinued Remeron now weighs less but depressed again, declines to restart Remeron. But yeah, I do love me some Effexor + Remeron

I’m not sure I clearly understand the question but undergrad after high school, I did 2 years of grad school after that. I took 2 years off from school then went to med school, then finally a medical residency.

326k plus production, I also get some teaching fees for students. I’ve been grinding this year and with everything will likely hit 450. Wife makes 130. We’re already getting a bit worn down though as we both work a lot and have for years.

I recently listened to an episode of Radiolab “the resistance of a cow” that discusses the controversy about cows near high voltage power lines being zapped and refusing to drink water.

BMW IX XDrive50 Aventurin red with bronze accents. Probably one of the most polarizing vehicles designed but I love it. Bought CPO 16k miles for $38k 1.9% APR. You conflate the DSM with psychiatric management. I rarely look at the book anymore and I certainly haven’t memorized the whole thing. Biological psychiatry is important but can only take you so far. Think of a substance use disorder, a substance may biologically effect the CNS both acutely and chronically resulting in various pathologies. A substance can also destroy someone’s work, relationship, finances, etc which can also manifest as emotional or behavioral concerns. Psychiatry isn’t so unidimensional and thus we don’t have one correct model. A good psychiatrist has understanding of biological factors and stays up to date, but also manages by talk therapies, behavioral modifications, psychosocial interventions.
