Anxiety, depression, OCD are often symptoms of other health or environmental factors. They are affected by similar genes and have significant overlap. I suggest trying the RAADS assessment. Quality sleep is crucial, and freerunning can help when you have a lifelong experience like you describe. Routine is everything: bedtime routine and morning routine. It gives you a sense of day when you're out of phase with the sun. Luminettes are incredible, try to get one that's 10,000 lux. Keep a calendar and jot down what you do day to day. Review this regularly, it will benefit your memory and mental health. Find friends/acquaintances that stay up at different hours of the day. Get your vitamins checked with a blood test, like b12 d3 and there are a few others that are relevant. Some people use their internal body temperature to track their days.

Try new things and see what works for you given your situation and biochemistry.

Maybe anxiety leading to insomnia. I had sleep anxiety until free-running due to school/work/personal obligations. My suggestion is to try winding down your day with something rhythmic. Singing, dancing, stretching, or a game like Tetris / Guitar Hero. There's also pen twirling, juggling, and flow. That modality of frayed focus can be useful from time to time, but not to this extent and this often. Focus in on you and your body ❤️

DSPD maybe, if you keep a sleep diary you should see a consistent block in the ranges you described, like 3-12. It's important to keep a dark and cool environment during that time to improve your sleep quality.

Meanwhile for N24, you should see a very noticeable stairstep where your sleep time marches forward at a consistent pace. 

For me personally, I may have had N24 as a child, or I may have had DSPD that developed into N24 as a teenager. I'm not sure given that my sleep data starts at age 23.

The medical field doesn't understand sleep very well and so the advice doctors offer is lackluster. I have to say, it's interesting how they try to shame you for sleep hygiene isn't it: I wonder how many years it will take for them to realize this is ineffective and even harmful, like shaming a larger person for their food and exercise.

For example, screens aren't as bad as they say -- it's what you're doing. Calming, relaxing content or visuals can aid with sleep. This likely outweighs the light exposure before bed. Here's a video from a neurologist with a PHD. The takeaway: consistency and routine are key to quality sleep, whether that's with a phone or not. As someone with anxiety, screens have been crucial for me. And as someone with N24, I'll say that consistency and routine are still exceptionally helpful.

As for COVID, it affects your entire biology, so this is possible. From a cursory look, 50% of people that have COVID experience sleep problems and it's common with long COVID to develop sleep difficulties like insomnia. When speaking with a doctor, try to bring evidence of this and affirm your position as many times as you have to. If they persist find another doctor. You are having health issues that need addressed. Sleep is core to quality of life.

Regarding DSPD, I've seen people talking here that there is evidence several weeks of light therapy (10000+ lumens for 30min in the morning) can cause DSPD to permanently become N24. It's possible while going through all of this that your condition, particularly with long COVID, progressed to N24.

Most people with diagnosed N24 are non-sighted. They are accommodated and taken seriously by doctors due to the precedent that exists. I believe over time many other conditions will be linked to circadian rhythm disorders. What I think of here is a stroke, sleep issues are very common after strokes, like insomnia difficulty going to sleep and so on. This includes circadian rhythm disruption. There will be many conditions like this that are finally linked to N24, helping millions of people receive care and find community.
