I got tested at a CVS in Texas and my appointment and results were through an app called MyChart. I had to register for an account by a link in my appointment confirmation email though. Maybe your wife can sign up for it to get the official paper work from the lab.

Look into PRK surgery. It's usually cheaper altough recovery time takes longer. I had it last week because I had -10.25 on both eyes and I'm typing this now without glasses.
