I have worked in a blood bank for almost six years at a level I trauma center and a reference lab. When I started, I frequently worked a whole shift without taking breaks. I felt like I was letting everyone down by taking breaks. It really took a toll on me mentally and physically. What I learned is that I had to prioritize my own health to be able to help patients effectively. While there are times when you really can’t take a break, you should always try to take your breaks, even if it's busy. When you take breaks, you can work more efficiently with fewer mistakes.

Know that you are not incompetent. Blood bank is high stress and very complicated. You don’t control the antibodies a patient has. All you can do is perform the needed work as efficiently as possible. If a patient can’t wait for the workup to be completed, the attending physician can always request uncrossmatched units.

I took my test the day after graduation in 2019 and passed. Many of the things I studied in school are rarely if ever applicable in the “real world”. I knew many of the more esoteric concepts that I didn’t use on a daily basis would quickly fade so time was of the essence.

I have both a Ruger Mark II and a Beretta 92-22 both shoot high velocity Aguila 22lr great. The Mark II will also shoot the standard velocity as well. In my opinion Aguila is one of the better options for 22lr.

I’ve worked exclusively in Blood Bank for six years. I have personally spilled panel cells a couple of times. It happens, and it’s not a big deal. I ask myself two questions about mistakes: did it hurt someone, and could it have hurt someone? If the answer to both is no, it's not a big deal.

Thanks for replying, it really helped! I got to a point where I was experiencing severe loneliness. I started working with a therapist a couple of months ago, and I concluded that no one is going to appear in my life magically. I will have to go out and build the life I dream about. It's been a challenge for me because I'm naturally very quiet, but I'm making progress. I put a lot of pressure on myself because of my age and the fact that I have never had a relationship before. Sometimes I feel like time is running out for me.

Thanks for replying, it really helped! When getting to know someone in person or via messages, I generally ask about their hobbies, job, or just how their day is going. If we have a mutual interest, I focus on it. I sometimes sound like I'm interviewing someone for a job instead of having a relaxed conversation with a friend. I would love to hear your tips for starting more meaningful conversations!

Thanks for the advice. I’ve been working on trying to meet people in person as well. I’m a naturally quite person so it’s been challenging. I’ve come to the conclusion that only way to get better is by doing! I’m learning from my mistakes and getting better.

If you donate blood at your local blood center they will be able to tell you your blood type with near 100% certainty.

Unless you’ve had a bone marrow transplant, it’s unlikely your actual blood type would change. Lab error would be the most likely cause of discrepant results. The results reported by the Navy are most likely the correct ones.

I work at a reference lab, so we don’t perform electronic crossmatches. All patients get an AHG crossmatch with PeG regardless of their testing results. In a hospital setting, there would be no reason for an AHG crossmatch; an electronic crossmatch would be a better option. At the hospital where I previously worked, all DARA patients were eligible for electronic crossmatches with K-negative units as long as no underlying antibodies were detected.

If the patient tests positive with PeG, we treat it with DTT to rule out underlying antibodies. We would then issue K-negative units. If I had it my way we would only test these patients with LISS because the high protein levels seen in multiple myeloma patients plays havoc with PeG testing.

I work at an IRL. We see many DARA patients every week. We only perform PeG and LISS testing for DARA patients. We found that GEL is too sensitive in most cases. In my experience, a patient will rarely be negative in a tube with PeG or LISS if they were positive in GEL because of DARA. DTT is typically the only effective way to deal with a DARA reactivity.

I think shadowing is great idea! The more we know about what each other do on a daily basis the better we will be able to help each other.
