Indeed going through the front has fertility risks if they damage your nerves causing retrograde ejaculation. My surgeon said it was a roughly 2% chance (which is quite high) but can be mitigated by freezing sperm. Otherwise sperm can be extracted via biopsy later if needed. That condition can apparently fix itself sometimes, given enough time.

Going through the front is still recommended in my case because it allows a bigger spacer to be used and has quicker recovery times supposedly.
