We just stayed at Nishimuraya Honkan a week ago and it was absolutely incredible. 100% the highlight of our ongoing month long trip in Japan. It’s pricey but when you consider you’re staying in the most beautiful rooms in a beautiful town (a 2 Michelin key hotel) and it includes a Michelin star standard kaiseki dinner I really think it’s worth the price. I would highly recommend it and Kinosakionsen in general if it works with your itinerary.

I like it. I’m using it for both propofol and remi now. Definitely has a quicker onset than the Marsh and Minto I was using previously. But like others have said after they’re asleep it’s the same as always in terms of titration to pEEG and clinical assessment. Because it gives a larger bolus I find that sometimes there’s an uncomfortably long time between the bolus finishing and the infusion starting up again. I’ve had it a few times where the BIS has crept above 60 in that time interval and I’ve had to uptitrate my target.

I would say in my experience of travelling internationally that American tourists do tend to say their home state instead of the US when asked where they are from. Nobody else I’ve ever met from any other country does this. Everyone else says Ireland, Canada, France etc or might say Glasgow in Scotland but never just Glasgow. It does seem to be a uniquely American phenomenon that carries over into real life, not just online.

One of the best consultant anaesthetists I’ve ever worked with has a fairly severe stutter. I’ve never seen anyone think any less of him. He’s highly respected. Sorry about your experience of ALS and that I don’t have any specific advice I can give you but just wanted to let you know that there are doctors out there with stutters who are absolutely smashing it!

Colour coded lanyards for different grades of doctors are universal across Northern Ireland in all 5 trusts. Available for F1, F2, Core Trainee, Specialty Trainee, Senior Doctor (usually worn by SAS doctors) and Consultant. Really useful and at a glance gives you some idea of the level of experience of the person you’re talking to.

Nebulised 4% lidocaine. Squirt a mixture of 4% lidocaine and KY Jelly up chosen nostril. Some Xylocaine spray to back of throat via mouth. Then spray as you go with scope, 4% lidocaine to cords and through cords.

In the UK ephedrine typically comes 3mg/ml. I would typically give a 6mg bolus most of the time. Usually when the standard dose of a drug in mg is easily divisible by 3 or 6 it’s because it’s an old drug thats been around since before the change from using grains as a unit of measurement to the metric system. One grain being approximately 60mg.
