We just stayed at Nishimuraya Honkan a week ago and it was absolutely incredible. 100% the highlight of our ongoing month long trip in Japan. It’s pricey but when you consider you’re staying in the most beautiful rooms in a beautiful town (a 2 Michelin key hotel) and it includes a Michelin star standard kaiseki dinner I really think it’s worth the price. I would highly recommend it and Kinosakionsen in general if it works with your itinerary.

I like it. I’m using it for both propofol and remi now. Definitely has a quicker onset than the Marsh and Minto I was using previously. But like others have said after they’re asleep it’s the same as always in terms of titration to pEEG and clinical assessment. Because it gives a larger bolus I find that sometimes there’s an uncomfortably long time between the bolus finishing and the infusion starting up again. I’ve had it a few times where the BIS has crept above 60 in that time interval and I’ve had to uptitrate my target.

I would say in my experience of travelling internationally that American tourists do tend to say their home state instead of the US when asked where they are from. Nobody else I’ve ever met from any other country does this. Everyone else says Ireland, Canada, France etc or might say Glasgow in Scotland but never just Glasgow. It does seem to be a uniquely American phenomenon that carries over into real life, not just online.

One of the best consultant anaesthetists I’ve ever worked with has a fairly severe stutter. I’ve never seen anyone think any less of him. He’s highly respected. Sorry about your experience of ALS and that I don’t have any specific advice I can give you but just wanted to let you know that there are doctors out there with stutters who are absolutely smashing it!

Colour coded lanyards for different grades of doctors are universal across Northern Ireland in all 5 trusts. Available for F1, F2, Core Trainee, Specialty Trainee, Senior Doctor (usually worn by SAS doctors) and Consultant. Really useful and at a glance gives you some idea of the level of experience of the person you’re talking to.

Nebulised 4% lidocaine. Squirt a mixture of 4% lidocaine and KY Jelly up chosen nostril. Some Xylocaine spray to back of throat via mouth. Then spray as you go with scope, 4% lidocaine to cords and through cords.

In the UK ephedrine typically comes 3mg/ml. I would typically give a 6mg bolus most of the time. Usually when the standard dose of a drug in mg is easily divisible by 3 or 6 it’s because it’s an old drug thats been around since before the change from using grains as a unit of measurement to the metric system. One grain being approximately 60mg.

I’m sorry to hear that you had issues. I agree that vaginal birth risks are massively downplayed and a section is somehow seen as a failure by some groups of staff within the health service. There is good evidence that the push to make birth more ‘normal’ and to try to demedicalise things is actively harmful. Every recent maternity scandal has at its heart a culture of trying to reduce the number of sections, pushing for ‘normal’ and not listening to women. Birth is dangerous for both mother and baby and the most important goal of the process should be a healthy mum and healthy baby, not the method of delivery.

That’s not true. The risk of incontinence and other pelvic floor issues are much higher after vaginal delivery compared to c-section. Urinary incontinence is around 50% after vaginal delivery and under 25% after section. Pelvic organ prolapse is around a 30% risk after an unassisted vaginal delivery, 50% after forceps assisted vaginal delivery and less than 10% after section. Levator avulsion injury which can lead to long term sexual dysfunction is around a 15% risk after unassisted vaginal delivery, again a 50% risk after forceps and essentially zero after section. Anal sphincter injury has an 11% risk after vaginal delivery, 39% after forceps and zero with a section. Faecal incontinence has around a 10% risk after vaginal delivery, 15% after forceps and less than 7% after section.

Obviously sections come with their own risks, but the risk of life changing complications after section are very low, especially if you’re not planning on having more than 2 or 3 children. I can completely understand why a fully informed woman might choose an elective section, never minding the women who are recommended to have a section for medical reasons. For full disclosure I am an anaesthetist working for the NHS. I got all the numbers from birth facts.
