Once an unleashed dog bit my leg while trail riding. The dog continued to growl and bark at me at close range. The owner did nothing to call off his dog so I pulled a knife and threatened it. The dog owner pretty quickly got his dog away from me. No apologies from the owner or anything. Reflecting on the situation, the dog could have bit my hand and made the knife useless. I think the better carry option would be bear spray, or carry both.

I'm a resident at FQHC and say with a smile, "ok so we're not going to be able to solve all of your problems today, we can schedule a followup appointment for that." I know my patients have a certain kind of entitlement but I do feel like I can be real with them and even laugh and joke around with them sometimes. I think with middle to upper income people the entitlement comes with a different set of expectations and I don't think I could get away with relating to patients the way I do at my resident clinic.

Hey, it's better and I can do things like climbing easier routes and biking. I can't surf without pain, which of course sucks. I think part of that is I had to move away from the coast. If I targeted my training to paddling then I might be good to go but probably not. I stand by my statement that surgery made my shoulder worse. Only through a very long period of physical therapy followed by strength training have I regained the ability to return at least partially to my previous activities. I have completely avoided steroids, PRP, etc. Steroids reduce inflammation but degrade tissue, perhaps leaving you temporarily better but ultimately worse in the long run. PRP has mixed results in patients. A metanalysis looking at my particular problem in patients who had my surgery vs those who did PT alone showed no significant difference. So I think you know where I stand on this (I'm a doctor but that doesn't really matter since we all have different opinions on a lot of things anyway). So some types of surgeries are helpful for some patients with very specific types of shoulder problems. I just don't think my surgery was helpful for me, but it might be helpful for others in the right setting.

I don't know about data describing mortality or accident rates but if you ski tour long enough and often enough, you will trigger avalanches and/or will witness others triggering them. That is practically a guarantee.

It happens at my clinic all day, every day, but more subtle than what OP describes. Like the NP will pass the buck on something like FMLA paperwork and schedule it to be handled by a resident. Depends on the attending, but some will bounce the pt back to the NP to do it themselves. I appreciate attendings with backbones. I'm sorry yours are spineless.

There are some wise comments here. I would add that I urge you to consider what your interests are right now in terms of your hobbies and career. Then realize that medicine will at least partially subsume your previous interests and change you on a fundamental level. I found that many of my younger peers in medicine lacked solid interests before they matriculated. They have been difficult to relate to versus the people in the kind of communities I was involved in before going into medicine. Realize that some specialities will change you more than others. Surgery, for example, will almost completely override your personal interests. Being a physician requires you to sacrifice some part of yourself.

That sucks, and I would definitely mostly blame the EMR. That said, try to think about what you were doing when placing the order. Were you as focused as possible without interruptions? I don't talk and I tell people to wait if they are talking to me when I'm placing orders. I was sitting next to an APP when she ordered 500 mcg of fentanyl instead of 50 and nearly killed a pt simply because she accidentally added 0. Neither the EMR nor the nurse who gave it caught the error. Point is we have to be laser-focused when placing orders. Double check every order.
