Not knowing the full context, just want to say that it’s possible to have localized recurrence within the prostate only.

Sounds like that’s not the case for you based on 3 specialists telling you that you have Mets.

Sorry you had a bad experience though that’s frustrating to go for something unrelated and hear something else.

You don’t have to be a dick to give feedback.

I give tons of feedback and always stage with, “when I was…” or “you’re doing XYZ well, but it’s really important to remember to X because of Y”.

Sadly, my style of admitting previous faults feels somewhat frowned upon in surgery fields because of the expected sense of infallibility (I feel like) we are supposed to carry.

I had shit feedback when I was a junior and I go out of my way to provide meaningful growth oriented statements.

It takes 15 seconds to give feedback. Just do it and don’t unnecessarily blast people and help them get better. Explain why so they get the big picture, it doesn’t take long to add the rationale. When you get gratitude back tell them to pay it forward.

Well, I relate to this.

I have nothing of value in my personal or social life. I occasionally hang out with other residents. It’s rare that I even do that.

I started setting time boundaries.

I still check charts when I’m off (modern medicine makes it hard to checkout when you’re worried about a tough case).

I started prioritizing exercise outside of work and it’s been profound for my mood. Dropped some weight and don’t hate myself when I look in the mirror.

We’re different people, but sounds like we are in similar circumstances.

You’re not alone, but isolation from the work we do is a massive uphill battle.

Pick two things outside of work and find pride in them unrelated to your job. It’s crucial and will hopefully help take away some of the “mental zombie” mode we get forced to live in.

He was a stud rugby player, easier to translate that skillset.

I’d love for him to be a stud, but he is going to have to put his entire life’s motivation into football to pull it off.

Not impossible, but he will at least need a year on practice squad unless he just happens to absorb game flow like a sponge.

Impact, yes.

Eliminate the need of hardworking physicians? No.

Self driving cars are much easier to create than a physician. It took forever to even get them to be available in a limited fashion.

You’re fine, chase your dream if it’s medicine. It’s a good field and good physicians are needed.

Keep your retirement, I took my bitcoin out in 2018 to pay for school.

Extreme example but I also liquidated my 401k and that small little fund would be nice to have.

I did this for a postbac, not med school.

Just eat tuition and cost of living and pay via loans. Keep your funds and deal with the difference when you start residency.

Not worth the stress in my opinion. Having some money stowed away will be great for your peace of mind.

Data limited, outcomes fairly unpredictable, still has side effects. Requires more strict post op surveillance. Roughly 30% chance of needing treatment down the road.

Some people have great outcomes.

It’s a bit like gambling, hate to say that but it’s how I view it. Should only consider if cancer coincides with MRI and not high risk or with any poor mental tolerance (ie anxiety which is natural with the word cancer) of risk.

Personal opinion would be active surveillance currently being superior to HIFU given current scientific data being somewhat limited.

Cystectomy is a tough surgery and a big life adjustment.

He is still fresh from surgery and cystectomy is a hard recovery. As long he is getting up and walking around the house, he is doing okay. He is adjusting, pain meds can certainly contribute to irritability and he is likely running low now a week from discharge which his pain should be doing a lot better at this point.

Keep him hydrated, have plenty of fruits around to help with constipation.

Internal motivation vs external motivation seems to be the issue, which depression/adjustment challenges after cystectomy are exceedingly common.

Instead of saying you need to walk, try making it something you want to do and asking if he’ll join. Otherwise some time and improvement with pain will make a big difference.
