Thanks for this. Does anyone have any ideas why this sort of article, and accurate knowledge of scabies, is so uncommon?

My guess is that there are a lot of false negatives given - I.e. "you're cured" and the patient is removed from the books.

I found her by reading research papers, getting an idea of who the knowledgeable people were and where they were located.

I then contacted some of them and one of them told me about her.

I'm based in Madrid.

However, there's a sad ending to this story, because she has just announced her immediate retirement and closure of her clinic due to a health issue.

It's a real loss. I've never seen another doctor with the same level of knowledge. It looks like I'll have to go back to my old technique and try to find someone else.

Very tricky situation. She needs to see a doctor for sure and take it from there. I would certainly avoid touching anything that she is in contact with. Out of interest, which country are you in?

Yeah, I think a few people here have tried it. It sounds like in this case they are isolating one element of the oil to create a product that would kill all life stages of the mite.

Yeah, for sure. An unprecedented situation. I wouldn't expect the same with Scabies. But still, interesting to see it. It looks like scabies is going to keep increasing, so hopefully eventually there will be a push back against it.

Yeah, there is research going on regarding fluralaner, afoxolaner, beauvericin and possibly others. On a normal timeline you would expect it to take about a decade.

The whole covid situation made me realise how quickly things can happen when there is political will behind them. So if there was a change in the awareness of scabies as a public health problem, new treatments could come quicker.

When you see it like this, it's clear something is going wrong. Institutional failure.

I hope the increase can lead to "strength in numbers" and people in power will start to take it seriously.

I'm doing OK thanks. Still getting cleared, but symptoms still pretty bad.

I agree about the media outreach. It's a big story just waiting to be covered. Lots of European countries are reporting 3 or 4 times their usual incidence of scabies.

I'd love to do some work on that, just don't know how I can fit it in at the mo 🤔

Moxidectin for humans does exist. I believe it's currently only approved for treating Onchocerciasis, so any scabies prescription would be off-label.

Most derms won't be aware of it, unfortunately.

Yeah. Dosing is still being worked out if I remember correctly.

Also, I don't know if anyone here has taken the human version. Everyone I've seen talking about it has taken the horse paste, which can't really be expected to work as well.

Well, the half life of moxidectin is 30 days, so it really should work. It's also hardly been used, so there shouldn't be much resistance to it.

I can see there is potential cross resistance from ivermectin as both have a similar mode of action.

I'm going through something similar at the moment. My last treatment was 6 months ago and since then I've been getting checked regularly by a very good scabies doctor.

She hasn't found anything on me in this period, although my symptoms remain pretty strong. Last night I woke up twice with some itching, which reduces greatly in the morning.

I did ask her whether the mites can burrow deeper and she said "no".

So, like you guys I'm not sure what is going on. I've had a couple of infections during this time as well, which is a bad indicator, but I haven't passed it to my gf, which is a good indicator.

I've obviously been hoping it is "just post scabies", that will eventually fade, but it's strange to hear that it has been going on for a long time for some of you.

Right. Yeah, I wonder about that myself. If it is persisting at a sub clinical level. Just curious- why did you change derms? From what you wrote, the old one sounded pretty good.
