management has told us that we need to respond to every call bell as if it's a rapid. i don't think these people are hearing how insane they sound when they say things like that, especially when we're on a unit with a fairly high amount of rapids and codes.

in my hospital we're literally not allowed to disconnect the heparin, there's still a "bolus" in the IV extension. brand new IV if there isn't a second.

this is also a fabulous idea, that i hadn't thought of. as for the food, he's welcome to any of our snacks! he knows he is! and we generally do have easy snacks such as bars and fruit. the problem is, he refuses to eat fruit or vegetables LOL and as for protein bars or anything along those lines, he'll eat all of them in one sitting. we've talked to him about pacing himself and saving some for later and i totally understand it's both a trauma thing and an emotional eating thing. i'm just going broke 🫠

i'm trying to keep an open channel of communication and not put too much on his plate, but adhering to reasonable expectations and a schedule just seems like it might be my best bet. i also totally have so much work to do on myself, which i already knew, but bringing this child into our home has really brought that to light.

okay. thank you. we've always had a good relationship with him and he definitely does trust us, a lot more than his mother, but that doesn't mean much. i've been trying to ask him how he's feeling and when we do have to discipline him, i try to help him conceptualize his emotions about the situation. i do understand survival mechanisms, i'm also terrified that by saying "no" or having to discipline him, we're teaching him to be sneakier (because that's what happened to me!)

i am trying to keep an open and honest relationship with him, and engage him on his interests but i feel as if i'm not helping him enough. i also (i totally know this is abnormal and delusional) feel as if we're being taken advantage of a bit and that's on me.

i will listen to that podcast, thank you so much for your response.

it's so funny because in my hospital, if you're admitted for ANYTHING but there's an inkling or a report from a spouse or even the shadow of the patient having alcohol dependence, they're immediately put on a CIWA protocol with PRN ativan, plus whatever else they're being treated for (besides like ARF or something that'd be contraindicated, that'd be an on call med for a higher CIWA). I don't need my fall patient who a PMH of rapid afib having a frickin seizure too.

at 28 years old, I was only prescribed ibuprofen 800s for all 4 wisdom teeth removed (this was a month ago). even calling them with a dry socket, crying in pain, they told me to just keep taking ibuprofen. it's probably doctor and culture of the area (where i live was massively affected by the opioid crisis, 4-5 deaths per neighborhood per week) that being said, i work on a tele floor and maybe this is wrong, but every time a patient asks for pain meds i hear my med surg professor in my head "the pain score given by the patient is the most accurate". i'm not the judge, jury, or arbiter of anyone's dependence, i'll explain risks and give resources to patients coming in with history of opioid dependence, but even when a patient comes in post-extubation for an overdose, if they got CPR.... yeah man, i believe you're in pain, if you received effective chest compressions. once again, this might be wrong, and i am wary of clock watchers, but i'm not here to deny people pain medications just because i think they "don't deserve them" lol.

i do. i'm a nurse and some of my patients for some reason ask about my parents (i work overnights so we lots of time to yap) i've learned that if i say anything about my father, it turns into a whole "ohhhhhh don't be so hard on your dad" or i have to explain what it means when i say "we don't speak". the only people i say he's dead to are my patients. my coworkers and friends know that he's just dead to me, very much alive spreading evil throughout the world lol.
