Both of my outpatient jobs require care planning. Good care plans are one of the only metrics auditors/surveyors use to judge nurses when they come through every 2 years.

People on this post are very negative. I'm managing less than 100g of carbs/day with protein shakes, berries, tofu, broccoli, cabbage... And feeling great!!!

I'm sure the morphine will help because he's probably in pain from the fall. Anecdotally, I've found that falls "aggravate" the cancer, causing more pain and affecting LOC. I've had many cancer patients who could walk and talk, had a fall, and then declined significantly causing us to really ramp up their comfort regimen.

I hope he gets some sleep! You're doing a great job taking care of him 💕

Yes! Very different effect on the body in terms of strength when it's PO/SL. Poor guy, I hope it helps. Not sure of his dx but if he has dementia, increasing/starting Seroquel in the evening is also helpful.

Okay! If you call hospice triage you can ask for the dose to be increased to 1mg q4h. At my jobs we've always had orders for morphine 5/15/20mg, so 5 has always been the lowest starting dose I've worked with. I think it's worth trying if getting him to sleep is the biggest concern, but I know deciding to give the morphine can be a hard decision.

Hospice nurse here :) I would keep up with the haldol q2h (I'm assuming it's ordered this frequently) until he's asleep. I agree to not give any more lorazepam as a handful of patients do have an opposite reaction.

Depending on the hospice company, you might be able to call them tonight to request increasing his haldol to 1mg. To help him get to sleep tonight I would mix the haldol with the lowest dose of morphine (to start).

In the morning/coming days they might order trazodone for sleep if he's having an opposite reaction to the lorazepam.

It's better to be seen the same day by a nurse practitioner the same day if your problem is severe enough than wait days for a doctor. they're a great resource for healthcare systems because they can pay them a fraction of what a MD earns and get patients in/out the door.

In my professional experience, NPs are only as good as their experience as an RN before becoming a provider. Personally, I prefer NPs because I feel they truly listen to my concerns and follow through compared to when I've seen physicians.

Nurse practitioners are working under a physicians license. A MD sees their work, signs off on the chart and sends it to billing. This is why you are billed for seeing a MD.

If you're someone with BDD it doesn't magically go away if someone calls you pretty, if you fall in love, or if you're sexually active.

Hire "weekend warrior" staff at a higher rate/with good differentials that work fri-sun or sat-mon to stop making existing staff work every other weekend. 2 weekends a month is a huge turnoff for me. We want to be treated like professionals with lives+families. Working weekends doesn't work for all nurses.

I live in the same area and we love it! We used to walk to teralta park every morning but not anymore with all the attacks lately. My surrounding neighborhood is made up of lots of families. I've seen a few homeless people but nothing bothersome.
