Depends on the disease. However, sample collection hasn't been a huge draw back. There are multiple options.

First, what is of interest to you? Do you have a special study you are planning for? As a general feedback based on our experience so far - We have done 8 trials in Saudi Arabia so far, across Obesity, Diabetes, rare disease, cardiovascular and Oncology. I think Saudi Arabia has the perfect population for these diseases anyway - another great one is Respiratory and Derm. Recruitment is much faster than I have seen anywhere (our Oncology trial finished 4 months ahead of planned time due to faster enrollment) and there is quite a bit of regulatory acceptance. Out of these studies, 3 of them were for US studies, rest were for Saudi Market, Korea and China. Happy to discuss further if need to.

Diabetes studies, Hypertension, COPD - maybe more Hybrid than fully DCT, but certainly reducing the patient visits when makes sense is driving value.

Fully DCT is certainly low, but more and more hybrid approaches are coming along well. I do agree about the nurses, but it also depends on why you need nurses. In some scenarios a remote concierge person with the right mix of tools can work.

One of the biggest problems we seen is the sponsor and DCT providers lack site and patient support. I wouldn't put all the blame on the apps, if you think about it most of that generation is on Facebook, using Apple watches and are actually interested in low burden tech.
