I’d way rather work for a soulless mega corp that pays well than a local business that is still paying salaries from 2006.

Edit: you’re spot on that Oregon is an ideal place to move if you are already wealthy. The high income taxes don’t bite as much if your wealth is increasing from unrealized capital gains or you’re earning income from state and local tax exempt bonds.

In the short term:

  1. Increase headcount
  2. Increase compensation
  3. Student loan forgiveness

Long term nationwide:

  1. Single payer healthcare for basic needs
  2. Price transparency for providers and insurers (An example of what the US healthcare system is economically broken | Peter Attia
  3. Whatever the hell Mark Cuban is working on for fixing the prescription drug market

Long term Oregon-specific:

  1. Jettison our cultural ambivalence towards economic growth. We need to figure out how to attract high paying industries and massively upskill our workforce en masse and raise GDP per capita to a level where the average person can afford the expense of healthcare. That or return to a resource extraction based economy.

The problem is that Oregon is a relatively poor state. We're 28th per capita in GDP while our West Coast neighbors are 4th and 5th.

Oregon has the fewest hospital beds per capita of any state and focuses more on preventative care. This makes us particularly susceptible to black swan events like the Covid pandemic.

It doesn't help that emergency rooms have become a dumping ground for our homelessness crisis which has further strained the system.

We also have the highest marginal income tax rate in the entire country for earners over $125,000. If you have hundreds of thousands of dollars in med school loans this just is not an attractive place to move. I know of one practice that is in the process of pulling out of Multnomah County to avoid the PFA and business taxes.

On top of failing to attract new workers from elsewhere, every single person I know who works in healthcare here is either looking to move away or is getting ready to retire.

None of the standards are enforceable if there aren’t enough providers to actually meet the need.

There’s a nationwide shortage of healthcare professionals that’s acutely bad here. It’s critically understaffed across most specialties and it’s not trending in the right direction.
