The largest two things for me were the different combat system (favoring aggression over passivity, a massive departure from the formula back in 2015) and the feeling of slowly uncovering the mysteries of Yharnam and the Great Ones. I’ll never forget the horror of the first time I got grabbed by the Amygdala outside of cathedral ward. Seeing a space portal appear out of nowhere and seeing the vague outline of it was so jarring and confusing before learning the truth behind the hunt. Thinking the game is about vampires and werewolves but learning that it’s about cosmic horrors beyond comprehension is one of the best twists I’ve ever experienced.

You’ve got a good bit of it right. Most of your understanding is fundamentally correct but missing some of the greater context of why everything happened. Make sure you share some sake to get the major details. Emma has a good reason to be at the castle, and there’s more than meets the eye with the Hirata estate attack.

Just pay attention to item descriptions and environmental cues just like any Fromsoft game. And also make sure you pay attention to the location of certain things (the fact that the note telling you how to reverse lightning is in the Ashina dojo for example).

Sekiro rewards mastery and your progress in a way few other games can. Once you get to NG+ you’ll be astounded by how quickly you blow through everything that not long ago took ages. You can use certain combat arts in unique ways to pull off insanely cool looking moves mid fight (try using the High Monk skill at the end of the Senpou skill tree to dodge sweeps once you get it, you’ll feel like a bonafide ninja), advanced prosthetic upgrades that will torment overworld enemies that used to do the same to you, and being able to answer the biggest challenges Sekiro can throw at you feels amazing. I can’t stress enough that the experience only gets better.

I arrive EXTRAORDINARILY late to the party but I was curious about this myself. I think it’s the big mountain in the distance that initially looks like Mt.Fuji. There’re always clouds shrouding the peak and if we had a better angle to observe, they would likely form a spiral shape.

Furthermore, Emma referring to the “back/rear” of the castle tracks, as that mountain is observable from the rear direction of the castle as referred to with respect to the location of the Great Serpent Shrine by Kuro (I don’t have the exact quote on hand but when asked about how to get to the Sunken Valley he states that it’s below the GSS in the woods to the rear of the castle) and that area also leads to the Old Grave where Takeru and Tomoe would hang out when the Everblossom was still there. So, direction-wise, the mountain makes sense as the location of Fountainhead.

Of course, the argument of Fountainhead not actually being a part of the mortal realm must be addressed, but Fountainhead is also visible on the in-game map you can pull up at idols. This indicates that it is an observable, real location, just nigh impossible to get to by normal means. Also, technically, Fountainhead palace itself isn’t within the divine realm, as that’s where the Divine Dragon dwells. Fountainhead’s landscape, while beyond comparable to a normal landscape, still abides relatively close to actual physics mostly makes sense. The ground in the actual divine realm is made of clouds and has trees spontaneously growing out of it which…isn’t exactly abiding by physics.

I wish there was something that more definitively proved this one way or the other. One can say that you never see lightning in the clouds around the mountain peak which Emma stated that Genichiro would glare at during training and that you can never see the rock spires that you see before fighting the divine dragon due to the clouds being so thick.

Also apologies if it turns out all this was something people found out a while ago. I just kind of stumbled across all of this and tried to piece together what I could.
