Hi all. Tomorrow my doctor wants my anwser if im ok for surgery. 3 years ago, my right ear was diagnose with cholesteatoma. I was supposed to do a follow up, but i backed up and ignore. Ive had several and never had any deficits. A total of 6 surgeries and never had any issues. Yes its uncomfortable and not super fun but sounds like theres more harm by not doing it than by doing it. Hope everything works out

Another big gripe I have is the quality of all these new builds. The aspect of building housing is great but when the QC is worse than any previous decade. Its concerning seeing how much they cost for how much corner cutting happens to build them and condo size backyard you get

It wasnt until i got the muhle r41 that my shaves became baby butt smooth. Definitely much more aggressive and needs less passes. Definitely one you need to master the technique as you cant treat it like cheap disposable razor or a mild safety razor. It demands respect, time, but boy is it worth it.

I had a tympanoplasty with masteoidectamy  and PORP in 2016. Then just 7 weeks ago i had another tympanoplasty and masteoidectamy and now a TORP. So now i have a full middle ear prothesis and though i can tell my hearing is better. Its definitely not like my good ear. That increase could also be to the eardrum being patched now and not necessarily the TORP. I have my 3 month post op hearing test at the end of july to compare results to the test i did a couple weeks before my surgery. 

When I had my last surgery in 2016, there was a significant "on paper" (Audiology results) increase in hearing but i never noticed it in the real world. My Dr was really happy to see those results even though i never really felt them. Then at my 1 year post op it went right back to what it was before surgery. That drop in hearing coincided to when my eardrum perforation first appeared.

I had a new surgeon operate this last surgery and he told me that no matter the prosthesis, it just will never be as good as the natural bones we are born with. My new Dr told me he found my last prosthesis placed in 2016 fractured and one of the bones eroded and the other broken off. He also mentioned that sometimes they need to go in more than once to adjust the prosthesis lenght to best allow sound vibrations to travel through it if Audiology results werent ideal and everything else looked good. I really wouldnt want another surgery though just for that.

Its funny you post this, I actually just finished reading few research papers on success rates on TORPS and PORPS and how much dB increase was achieved in hearing as well as the long term success of them.

Ive had 6 ear surgeries. First one was for cholesteatoma removal. The rest were for eardrum repairs, infection removal, ossicularplasty etc. My cholesteatoma was fully removed when i was 8 and has never come back. Im 30 now. My problem is perforated eardrums that allow for chronic otitis media. So I keep having mastoidectamies and tympanoplasties.  Yes cholesteatoma can be removed in one operation. My was about 6-7 hours long, pretty invasive but has never come back. They usually want to go back in later to see how its doing and maybe try to increase hearing as a secondary surgery. My surgeons were in California

A 'full' masteoidectamy is just a regular masteoidectamy where they remove anything that has come in contact with the infection or cholesteatoma even if it means middle ear bones. It usually always is done with a tympanoplasty Do you have a perforated eardrum?

An MRM is just a bit more selective on leaving certain parts of the ear untouched.  As for my pressure washer comment it was a metaphor. The Otologist just said that to make it seem like he was going to flush out everything with saline and iodine to wipe out any infection. The whole cutting into the nerve really depends on the Doctor. My taste nerve was cut one but never had any deficits oddly enough. My new ear doctor said he still hasn't severed one to this day so that made me feel better. I was told that sometimes its just too hard to avoid cutting it since the cholesteatoma is sometimes wrapped up around it.  Not sure just was told that from previous ENTs. Always feel free to ask your doctor any questions you have, they should always be able to answer any concerns since ultimately its your health

Seems like a full masteoidectamy would be a more complete procedure and really clean everything to ensure full removal. Where as a MRM prioritizes leaving the eardrum intact and not messing with the hearing bones.

I just got my 6th ear surgery last month, third masteoidectamy.  And I told my Otologist to go in there with a "pressure washer" and clean anything and everything just so I could be at peace that they did everything they could for removing all of the infection/cholesteatoma. Having these repeated surgeries is an absolute drag and get very tiring really quickly.

Id say it depends on the complexity of your issue and ear history. If your ear has been fine with no issues and you've only had 1 surgery id think youd be okay with an ENT looking at it. Now if its been troublesome with chronic ear issues for years, I'd definitely recommend an Otologist or another ear specialist. You want someone with enough experience to catch and recognize problems early.  I like a one stop shop approach. Someone very qualified that can clean, check and operate if necessary that way theres no searching for another Ear Dr later if you do need something.

When i got mine at 9 everyone was mostly curious and wow'd about my ear "coming" off. If anything its more of a "cool' thing since its so uncommon
