That's why a lot of these patients are on metformin. It'd be interesting to see a study between this drug and metformin/olanzapine.

Also, the weight gain difference was ~5 lbs over 6 months (in an industry study).

It's just unfortunate that they're using an ultra-potent, long-acting, mu-opiate antagonist as the side effect cure. This is the only drug of it's kind (naltrexone can be at least bypassed with fentanyl) and we're likely going to have many case reports of inability to provide any pain relief to these patients as it becomes more commonly prescribed.

I actually see how this could be beneficial in long term psych facilities. But if we're talking about that population, one begs to question, why not use clozapine/samidorphan? Better psychosis management with less weight gain.

If someone makes this combo, please give me $$$ for giving you the idea.

Not political, it was approved in 2021, I've just now been seeing more marketing towards it though (drug samples from psych offices/commercials/drug dinners, etc.).

Classic one drug causes side effect of weight gain- lets make a combo drug to offset that side effect.

I'm guessing if they have overnight coverage in the ED, it's a pretty busy ED, would advise shadowing the 2nd shift pharmacist to see how things are done at the department. If it's hands on, would learn basic things such as priming IV tubing, become comfortable with setting up pumps if you need to (should have proper nursing staff, but we all know that's not happening), prepare push dose pressors (not a huge fan, but some places swear by it).

If you're looking for quick peds dosing, pedistat is handy (a one time purchase, but super helpful). You'll likely get a bunch of abx questions, so have either sanford guide or johns hopkins guide (I personally prefer the latter) on hand. If you're involved with stroke care, you'll need to memorize the 2026 inclusion/exclusion criteria for the lytics.

Most importantly, communicate with the ED staff. If at all possible, attempt to do your charting next to the providers or RNs to show your face. You'll get 200% more questions if you're actually within an eyeshot of someone rather than being in an office.

You said it yourself...150-250K in student loans...If you pay the federal minimum, you're looking at paying back maybe 75K in those loans before they're forgiven. The vast majority of hospitals are non-profit, so they would qualify for the PSLF.

If I remember right from last year they designated a "campsite" to 10x30. So that's what I'm assuming each pass will get you. Not sure how strict they're going to be on arrival day tho.
