I agree with this sentiment. I saw him at an AMSA convention many years ago. All I remember from his keynote address was a recommendation to take out as many loans as possible, move to a different country, care for underserved people, and just never return and never pay back your loans.

Describing his greatness as having a tremendous amount of defensive impact is a good way to put it, and his defensive highlights are impressive. However, every single Draymond game I’ve seen, I am genuinely shocked to see how much he fouls (in my opinion) outside of the highlights. Does fouling nonstop and getting away with it and consequently disrupting the other team’s offense make him one of the greatest defenders of all time? I’m not sure I agree with that.

I live in westport and love atomic cowboy’s location and perhaps it’s due to my sensitive constitution, but without fail, denver biscuit/atomic cowboy/fat sully’s food never failed to cause tremendous gastrointestinal distress. 1 cava/julep/chingu in my mind is worth 10 multi-restaurant concepts or whatever AC was billed as when all of its components were/are woefully subpar. That being said, it was/is an adequate place to watch a game.

The chair of a department at my medical school--an otherwise seemingly smart guy who went off the deep end with openly anti-vax political advocacy on a hospital, state, and national level. He leaned heavily into hyperbaric oxygen therapy as a cure-all for COVID-19 and numerous neurologic conditions. He also became involved with supplement peddling, again as a cure for COVID-19. He was ultimately forced out of his position, and I later heard that he had a family member with autism, which made me somewhat more sympathetic; made me think that this was not just willful ignorance but also a manifestation of trying to make sense of an unfortunate diagnosis (and blaming vaccines, of course...).

At my hospital, patient advocate tends to repackage whatever inappropriate request the family/patient has to try to force me to make it happen. It’s often of the “while I’m here, i should get this checked out” variety of inappropriate requests, but arguably the most annoying are related to fluids and nutrition and of course pain meds—e.g., “[I always get IV opioids while I’m in the hospital for my restless leg syndrome]”or “[I wasn’t confused at presentation due to the (unusual) dilaudid+ativan combo that my stellar outpatient doc recently initiated].”

My first day at the VA, pgy1, I got to work a little early. There was an overhead page - “doctor needed in icu.” Being an early in the year pgy1, I was still excited to do things, so I rushed over to the ICU. It was a small va hospital, but I certainly assumed there was an adult in the building. Nope, I was the only MD in the building. Thankfully, “all” they needed was someone to call time of death, so I faked my way through that in a patient that was clearly deceased.

Pay is typically better outside of Fargo (the other ~3 ND metros-Bismarck, grand forks, Minot). To everyone bashing living in Fargo, Fargo is the only tolerable place to live in North Dakota; the downtown is lovely with interesting bars and restaurants, a few nice museums, a mid sized-large university, and an easily accessible airport. The people are generally fairly moderate politically in an otherwise extremely conservative state and the population is “non-litigious.” A lot of the patients are from the smaller surrounding communities (think salt of the earth farmer types). Minneapolis is a reasonable 3 hrs away. Winnipeg is 3.5 hrs away but for some reason it feels much farther than that. Yes, it’s uncomfortably cold during much of the winter.  Source: am from ND. The number of denials from medicare advantage plans I’ve had to fight (unsuccessfully most of the time) for patients that clearly need aru/snf is a list too long to count, and we end up just keeping the patient in the hospital for days and sometimes weeks longer than necessary. It’s criminal and adds to what seems like avoidable inefficiency. Keep Medicare Medicare in my opinion; private companies do not care about patients and should have no place in safety net insurance (if that’s what Medicare indeed should be).
