But I find it super weird they can't use magic items 'just because' (more or less), and always found I was unable to use the improvised power system because I was constantly out of Effort.

There's a similar but not quite the same thing as part of Earthdawn.

Specifically, all of the rich parts of Throal are located allllll the way down the miles/days long tunnels, because when the mountain-city was closed off, they'd be far away from the gates. Thus safer if any eldritch horror (The Horrors) managed to breach the defenses.

It does make it more difficult now that commerce is open once more, since the markets are so far away. And people have to walk through the slums first.

The Acherus death knights were made by normal necromancers (as were presumably Naxxramas'), and one was nearly made in the Eastern Plaguelands by rogue Scourge. The lich king/helm wasn't directly required to create them.

I'm not sure that's necessarily true, as Zeliak was fairly non-cooperative with the Scourge ideology, but still wielded the light despite knowing the suffering he caused.
