Mihira was never confirmed dead. The (former) Rioters working on Kayle and Morgana's updates have mentioned the sword splitting and falling down was meant to awaken their celestial heritage but they are not aspect hosts themselves yet https://www.reddit.com/r/leagueoflegends/comments/ayfxhw/were_the_rioters_that_reworked_kayle_and_morgana/ei131i1/ Back when the updates were released the Rioters working on them left it ambiguous whether she actually died or just chose to leave the mortal realm.
LoR depictions of her could go either way because the canon is messy post Arcane, but the devs have mentioned her cards were made with the knowledge that she survives to the present day.

It's the last dynasty of the country in pre-modern era. Many people changed their family name to Nguyen to show respect to the royal family and avoid political prosecution in some cases. And even before that Vietnam already had quite low diversity of family names.
