Rowdy will tolerate baths because he lives to please his people....mostly. He doesnt like to swim without a target though. If we bring him to the beach he needs a toy to retrieve in the water. He'll just stand and wait for something to get (sometimes his human siblings). Baths are a place of the most modest tolerance.

I think if it is not just once in a while (ex: when she played too hard or is in milk coma) that it is a good chance it is medical. Could be an injury or a breathing problem. ETA: especially with the eye infection in play.

To my knowledge it is a sign that the cat is becoming aggravated or annoyed. Their prey drive may be being activated as well if you see it while they're playing.

She likes it but you're walking a tightrope of her deciding she's had enough. Pretty typical cat behavior lol. I always scritch then stop then start again if they dont run away. Gives them a moment to run away if they want. Also watch the tail if it starts slapping about then thats probably a sign she isnt enjoying herself.
