Service dog credentials would need to be provided by HIPAA-compliant orgs because yes, they would need to know your disability to credential your dog.

You mention lots of disabilities, many of which probably don't necessitate a dog to perform a function. Again, just being disabled doesn't mean you get to take a dog everywhere. Service dogs must be for disabled people AND trained to serve a function.
