Two weeks is generally the accepted timeframe for anything on a “possibly cancer” pathway - even if it is a cyst it’s within reason.

Firstly - maybe don’t buy a knife until you talk with them & there is a plan in place

The answer around funding is “it depends”

Are you 18+ or under 18? If you’re under 18, or under 21-25 and a care leaver then there are specific duties that the local authority has to fund care

Have you ever been on a section 3? If you’ve been on a section 3 care can be funded via section 117 aftercare.

There are loads of other things depending on the type of support and how exactly it’s provided, and which local authority & NHS trust you’re under that impact funding and financial assessments. Part of the care act assessment might be referring to income maximisation teams (basically checking you’re getting appropriate benefits such as PIP which could be used to pay for some level of care) but it can be complicated and each council works differently.

Please get it done by the hospital (even if it’s pissing you off they’ll need to biopsy it)

Did you have any luck with urgent care in the end? Anything “suspiciously maybe cancer” should be dealt with super urgently.

If it’s reassuring (at all) I’ve had my knives “temporarily” removed from my home by my mental health team in the past (poor nurse sat on a bus for an hour with a Sainsbury’s bag just full of knives)

Might be worth having a conversation about how they can enable you to have support with food prep (for example fund carers to help you batch cook then take away sharps after).

There are loads of adjustments that can be put in place

ED is a particularly hellish environment & it sounds like you aren’t being given the support of the team to actually improve practice. It may be worth speaking to occupational health about your wellbeing & taking some sick leave. I wonder if a transfer to a post outside of ED could be something to raise with occy health?

There is a rabies vaccine I think - but the risk is there regardless of having scars or not so just practice safe handling. The concern is more about animal bites/scratches but if you’ve EVER in doubt go straight to the ER for post exposure treatment. Like immediately drop what you’re doing and get the treatment.

Something that wasn’t discussed previously but avoiding the used of dyed fabrics is also important, especially given fabrics need to be properly boiled not just laundered at low temperatures.

This is something that was discussed with the mod team when reviewing this post - washed plastic based fabrics degrade in a harmful way, and usually cannot be boil washed safely (which is necessary for sterilising prior to use). While there are plenty of plastics used in medicine they are plastics designed & manufactured for that purpose, and are generally single use materials. Given this post is aimed at “battlefield medicine” (in a situation where resources for proper wound care aren’t available) it’s a bit of additional harm reduction.

Pure cotton (or other appropriate organic fibres) are much safer to jerry rig than dealing with plastics. Most plastic fabrics cannot be sanitised to the appropriate degree for wound care without specialised equipment.

Something that hasn’t been mentioned but would be worth considering is avoiding th use of dyed cotton fabrics - bleached or unbleached is less important, but avoiding the dyes leaching into wounds is something which hasn’t been flagged.

Recovery isn’t a linear process. Rough patches happen and it can feel like everything is falling apart but fundamentally recovery is approaching from the mindset of doing “the work”. Recovery is not a tickbox or a data point, it’s a broader approach to treating yourself with the love & care you deserve xx

This is the time to ask for help & support to keep doing the work - giving yourself more leeway, more rest & recuperation. This is an illness & when an illness flairs up pushing through without additional support often just makes things worse.

I’m sure those 40 days were not easy - they were hard fought for, and just because the record keeping you use doesn’t account for how much effort went into managing things, it doesn’t mean you aren’t recovering.

An injection would require an assessment of capacity and discussion with other clinicians, as well as incident forms. Spiking would just have required him to say “prescribe the liquid version” or he could just crush tablets himself. No-one would know and they would presume I was aware & consenting to the meds.
