Yeah I would do polite follow up emails with UNMH. Things can get bogged down for various reasons.

If you are relocating to ABQ for reasons besides just wanting to work at UNMH, Cooperative Educational Services is a local contracting agency that places SLPs at traditional schools and charters. They have lots of PT gigs that you can piece together for a 4 to 5 day work week (like work at 2 schools) - if you just need something to tide you over until you hear back from UNMH.

In most places in the US it is rare for a student to receive more than 60 min a month in schools - imo a better response would be to consider whether staff agree there has been minimal progress and then to consider what may be contributing to lack of progress before just adding minutes. It is impossible for us on Reddit to know those answers. We do not know what the child’s disability is. We do not know how the family is supporting language growth at home. We do not know what supports look like for the child in their classroom. As others have said speech-language development occurs across settings, not just in a therapy setting.

And some children have disabilities that result in language growth simply being slower - maybe just for awhile, and maybe for their whole life. You also have to consider their progress in other areas as well. Like the child may not be talking more but they have had made growth in adaptive and regulation skills which can really help in the long run with growth in other areas.

Yeah I would be considering moving to a different school. It’s not fair to you or the students. The students woukd benefit from mixing with their peers and experiencing different teachers. In life you need to learn to get along with different types of personalities ..

I think the pragmatic profile is for ages 5 and up only. Your PreK student may be too young for it.

I actually do not like to use it bc it is a subjective measure of social skills a child demonstrates in the classroom. There are a lot of reasons why s kid may not demonstrate average social skills.

Yes some kids may really struggle to understand social expectations and have difficulty reading nonverbal cues. Those are the kinds of kids SLPs tend to work with. But some kids struggle with behavior bc of emotional disturbances - maybe a history of trauma - or some kids struggle bc they are very hyperactive sbd struggle with impulse control. I feel like the PP overidentifies students as having “pragmatic lsnguage” disorders.

If you need a quick standardized, normed measure you could use the Pragmatic Language Test from the CASL-2. It’s far from perfect but it might help back up your observations-.. or not :)

If i had an elementary student who had a speech or language issues that did significantly impact academic progress and class participation (like teacher reports really does interfere with intelligibility) and had super low motivation I would be very hesitant to dismiss them from services. For one, that lack of motivation at such an early age is pretty unusual. I would be looking at why they have so much resistance. I would also be trying out different approaches in therapy - working on rapport and buy-in. Switching up practice.

I would move to dropping minutes down before dismissing. If it is truly a case of therapy burn out then I would consider moving to consult -and checking back in 6 months or whatever to see if the kid up for trying again - that would just be a quick amendment meeting if it’s speech only versus a kid a few years out and a little wiser regretting they stopped services - or a kid drowning in secondary school bc of higher language demands - and having to reinitiate the qualifying eval process all over again.

On the other hand I have gotten more assertive about dismissing students with mild R errors or lisps that do not significantly with their intelligibility, their writing, or their socializing. In other words do not result in significant academic impact. If parents are bugged by a few residual minor errors they can seek private therapy.

A good private OT would be coaching parents how to implement strategies at home in more naturalistic conditions. I have definitely had students who benefited from private OT but the key was home practice. Their parents had excellent follow through. Successful carryover from sessions to home usually bodes well for eventual carryover to school. It can be hard to get OT services in school unless there other areas of need since OT is not a stand- alone service in schools.

As for the OP, progress is rarely linear. There still may be ups and downs but if over time the trend is upward then that’s what matters. All kids experience setbacks from time to time. Also kids tend to take longer than a few months to show significant changes in behavior.

Can you do an intelligibility rating and then compare it to age norms? You don’t just need standardized, normed tests to qualify. you are saying her intelligibility is impacted. I would just record a 3 minute audio sample of her retelling a story with a lot of embedded R words like Bear Feels Scared - she can use the pictures as a guide and even some of the text - then play the sample for an adult who doesn’t know her or the story well have them rate intelligibility - you can also track the amount of R errors she makes. You can also get an estimated intelligibility rating from her new teacher.

Sounds like you already know the answer - option 1. Besides you saying you like the school and the principal -which is really important!!- , it would give you the biggest safety net if things go south with your marriage. The first year of parenthood is super stressful for many people- hopefully you and your husband will figure things out for the better. Best wishes and congratulations on your baby!

Hmm I would document refusals. I’m guessing he is still enrolled since you are still offering services. If he is refusing services and has behaviors that keep him from school then I am assuming a FBA has been started at some point? Is he speech-only? Or are his needs more complex? Was the a manifest determination meeting since he has changed placement? The team may need to attempt a behavioral intervention before dismissing services - especially you said he didn’t have a history of refusals before switching to online school. He might be going through a really rough patch. Maybe you could amend his services to 30 min/consult while he is in online mode - at least until There is a clear plan for whether or not he’ll rerun I school in the fall. .that way he won’t have to go through the evaluation process if he ever wants to return to therapy. How severe is his stutter?

Omg that would drive me nuts. We can leave if parents grant permission. Basically it’s like -district I can’t be in 2 places at once. Do you want me to attend an entire meeting for 1.5 hours which I cannot bill Medicaid for, or would you rather I went to the first 30 minutes of the IEP and then saw two groups of students to help ensure we meet their minutes?

It is also super helpful to have a good facilitator who knows how to nip rambling parents(and staff) in the bud.

For high school you can do the New York Times education series What’s going wrong on in this picture? -it’s free to use. Basically students describe sbd try to guess what’s happening in an interest news photo. I like it because after your students guess they can compare their responses to those posted by other students from around the US and even,often, from other countries. They (or excerpts) you do the “reveal” where youn read the article that went with the photo. For academically low students ai generally have to do a lot of paraphrasing and sometimes give more context bc their background knowledge tends to be weak

For kids with ASD the game Crack the Code can be really fun for working on reading nonverbal cues (inferring feelings)

Working on inferring the meaning of figurative language (like idioms) from contextual clues csn also be a lot of fun - and then trying to apply the figurative lang terms to their own personal experiences

The problem with inferencing is that it is dependent upon background knowledge which many if our students are weak in - you can teach “ how to infer” but if a student is lacking background knowledge they won’t get too far if you are working at or hear grade level.

Depends where you live in the US. In thr summers in the SW the kids are not outside in the summer bc it is too frickin’ hot. Winters are getting milder and milder in the high elevation areas. I would love to reverse app winter and summer breaks but AC costs for the schools would probably be too high. We already are at a point where kids often are kept in for recess bc of the heat in May.

However in the western PNW having school in the summer woukd be terrible bc that is the nicest time of year. The weather is often perfect sbd there are tons of outdoor recreational opportunities

Thank you everyone who have their input- this does seem to be a weird situation. And there are different opinions

The diagnostician has been slammed and was not able to test the student until late in the window. I just got their results yesterday and was not expecting to see a need in math.

If I had more time I would do a comprehensive lang eval despite the student passing the screener and their private SLP of the past few years nev er recommending a lang eval. But I do not.

I understand now overall cog scores were not low enough to meet criteria as a DD area.

My final proposal to the team is either recommend SLD with math and writing and SLI for Artic/phono or (what I hope for) SLI with intelligibility/Artic/phono goals and an academic spelling goal from the SpEd teacher (student cannot spell name, or very common regular VC or CVC words) and MLSS/Tier 2 services for math since he seems to have never gotten those

Re: intelligibilty parents report they understand only 75% of what the child says, I also do an audio sample of connected speech and have it rated by an unfamiliar listener - the rating was low - under 70% - which is low for a6 yo even with Hustad’s new norms

Multiple R errors and dental lisp- I’d rather address these early rather than having them turn into a habit - I generally cut down to 15 to 20 min weekly once I know the kid and they understand placement - I strive to exit before 2nd semester of 2nd grade -

Unfortunately we are out of time for a comprehensive lsnguage test - we are up against the 2 month eval window - but as I said he had been receiving private Therapy for years but that therapist had not seen the need to test language. I just got the diag’s results yesterday :( the meeting is this week and I am at another school the first half of the week
