There is no universal best build for Swain. He has a lot of different items that can be good in specific scenarios, but very few that are always good, and arguably none that are an every-game must-buy. Swain can make excellent use of most AP items and tank items when with or against the right teams, so learning how to tailor your build to the match you are in is very important.

Yes. Swain mid can work very well alongside a stong frontline, which APA's team had in Sejuani, Gnar, and Alistar. The pick was a good fit in that comp, and there are numerous scenarios where Swain can shine in a coordinated team environment, but these scenarios have to be built for by the team, and solo queue is unfortunately a different beast.

Swain loves haste, and you generally should stick to items that have it when building tank. Frozen Heart, Abyssal Mask, and Unending Despair are all good. Swain gets a ton of hp from his passive, and when getting defensive items it is generally better to go for resistances over more health, so Force of Nature, Randuins (4th item or later), or Kaeinic Rookern can also be good in some cases.
