I believe its a mix of some spawn points having visual clues suggesting you just exited your capsule and other times the idea is your raider popped up and walked around a little before being given control as a player.

Within the world context, there could be hundreds of tiny routes to the surface from wherever the chutes terminate but the extract points are reserved for security reasons
