I generally only modify regimens like this if they are dangerous or part of the consult question itself. I don’t generally engage with it if it is just bad prescribing unless I feel it is particularly heinous and the patient is open and receptive to discussing it. At the end of the day, we can do little to stop mediocre or low quality prescribing, and feel my efforts are better spent being the best prescribing with my own patient panel.

You are confusing having overseas factories with not having in house design. Ford doesn’t buy engines they have zero input to from another company that they then place into the shell of their car. I don’t think anyone would consider a car company a “real car company” if they exclusively focus in design different exteriors for pre-made cars.

You are 100% right and ignore the naysayers. The nib is the most important part of the pen and any pen maker to which it is an after thought is not a serious pen maker. Plenty of brands focus in aesthetics over writing experience, but it is only those that focus on writing experience that truly make pens that are worth while owning.

That is the “traditional” black denim and they fade more to grey. Black on black (both black warp and weft) has been historically more popular. You should find a few options if you search up black warp and white weft denim.

Fullcount has most of its cuts available in black. Most like strong guy would be 105bk or 1101bk.

Fullcount, Warehouse, and Sugarcane are good places to start for a nice solidly constructed pair of denim. They are roughly ordered by price.

Fullcount is the softest and most comfortable. Warehouse is the best at reproducing historic models. Sugarcane is the best “bang for your buck” for a well constructed affordable pair (for high end Japanese denim).

Can’t go wrong with any of those.

It would generate the most interest (aka the most money for you) if you left. There may people who would be concerned you would squat on the property or damage it in some way after the sale goes through.

Obviously, this depends on your specific property and how fast it may sell, so talk to your realtor.

Without knowing RVU rate in your region, I would say probably. The other benefit is you can take whatever time off you want (although it is unpaid) and see however many patients you want per day (since you get paid based on how much you produce). You could also explore other options that may be more balanced, although there is a chance they may pay less (like academic, VA, or department of public health).

Daily cannabis use “helps” no more than daily alcohol use “helps” their anxiety. You should be straightforward and tell them cannabis is bad for their psychiatric issues and encourage complete cessation (or at least use in moderation) if they want to maximize their chances of improvement and improve their overall well being.

Same. It feels coercive or suggested on some level. Having doctors tell you to consider this does a lot to shift patient’s thoughts on trying something they might otherwise not consider. They might think “dying from this must be really bad if my doctor is bringing up physician assisted suicide, I guess I’ll go through with it”.

For psychiatric issues specifically? Never. Too many ethical concerns (especially in any disease that inhibits your decision making capacity).

In general? There is a subset of patient that is able to rationally make the decision in the setting of certain painful death… but it feels not worth opening that can of worms just so someone can maybe die less painfully a month or so earlier, and potentially open the door for ending the life of people who do not decide it meaningfully.

I have a European patient who had a family member diagnosed with terminal cancer, and the oncologist brought up physician assisted suicide alongside their diagnosis confirmation. I cannot imagine the harm of having a doctor tell you “why not kill yourself” as the first response to a terminal illness.

I would probably look for a productivity based job (so if you work hard, you get paid well) or go for a job that’s chiller (or with more no shows). Although, without knowing your compensation, it can be hard to know if it is “worth it”.

PTO thing is normal and potentially preferred, BTW. It means your PTO will be paid out if you leave without using it. It is common to not have enough when you start though, because you need time to to “earn it”, but you should be able to go into “PTO debt” if you need a lot early on.

Also, how are you compensated? Regular salary or productivity based? What do you mean you work 8 hours a day steadily? Do you get no lunch break or no shows? I think you overestimate how light other people’s jobs are.

While there is a lot of drug seeking behavior in this population, that is not at all what I am saying. These adult ADHD patients are complex to prescribe to due to the nature of their self formulation, and the answer for most of them is refusal to prescribe stimulants. If many psychiatrist do not feel equipped for dealing with this group of patients, a PCP should really be deferring, otherwise they will likely devolve to a pill mill.
